Study On Evaluation Of Community Health Service Resource In China Under The Background Of Hierarchical Medical System

Background:Grading diagnosis and treatment is considered to be the critical process to solve the Chinese longstanding issue of difficulties and high expense in medical care,but also a key measure to deepen the comprehensive health care reforms.Community health service plays an important role in the provision of basic public health services and basic medical services.It's not only an important part of the grass-roots health care reform in China,but the realization of the first primary diagnosis in the grading diagnosis and treatment.However,with the development of community health services,contradictions have gradually appeared in the allocation of community health resources.There are phenomena of unequal distribution and wasting of resources,with the result that the medical demands of community residents can not be met.Therefore,analyzing the equity and efficiency in the allocation of community health resources can effectively reflect the status of Chinese allocation of community health resources and utilization and bring matters out in the allocation of community health resources,so as to promote the grading diagnosis and treatment smoothly and supply suggestions to improve the allocation of community health resources.Objectives:In order to respond to new trends properly,based on the equity and efficiency of resource allocation,this paper evaluates the utilization and allocation of community health resources in 30 provinces and cities in China and the three regions of eastern,central and western China,identifies problems in the allocation of community health resources and look forward to offering advice on the allocation of community health resources.Methods:This paper descriptively analyzed the overall situation of China's annual allocation and utilization of community health resources from 2011 to 2015 first.The Lorenz curve,the Gini coefficient and the Theil index were utilized on making a comprehensive analysis of the equity in the allocation of community health resources in China.From the perspective of resource allocation efficiency,the data envelopment analysis model was used on analyzing the allocation of community health resources from two of the horizontal and vertical dimensions.And the total factor productivity in the allocation of community health resources was decomposed to further analyze the main causes of efficiency.Results:The overall development of community health resources in China is steady in China,but some differences exist in the allocation of resources among the eastern,central and western regions.The physicians to nurses ratio is relatively low;the structure of community health human resources remains to be improved.The equity on the basis of geographical distribution in the allocation of community health resources should be further improved.Regional differences are the main reasons for the overall difference.The allocation of community health resources is inefficient and technological progress has an impact on total factor productivity gradually.Conclusions:The equity and efficiency should be combined insistently to coordinate the allocation of regional community health resources.The grading diagnosis and treatment should be vigorously promoted."First primary diagnosis" in community should be more generalized.The building of a group of talents in community health should be strengthened.The regional difference in the allocation of community health resources should be continuously narrowed down.
