Study On Treatment Of Tuberculous Accompanied With Anorectal Fistula

Objective: The purpose of this study was to investigate the clinical effect of the timing of anti-tuberculosis treatment and surgical treatment in the treatment of tuberculous anal fistula in pulmonary tuberculous patients.Methods: Clinical data of 52 pulmonary tuberculosis patients with tuberculous anal fistula from January 2008 to January 2016 in the First Hospital of Xinjiang Medical University and The Thoracic Hospital of Xinjiang Uygur Autonomous Region were evaluated according to the choice of operation time.Patients were divided into two groups undergoing surgery before anti-tuberculosis treatment(group A)and surgery after anti-tuberculosis treatment(group B).Clinical analysis was performed using correlation index.Result There were no statistical significances(P>0.05)between the two groups in the aspects of postoperative pain score(x2=0.087?P=0.768),age(t=0.231?P=0.389),ethnic(x2=0.391?P=0.532),duration(t=0.422?P=0.675)and recurrence rate(t=0.377?P=0.708).52 cases of patients with anal specialist examination detection rate was 75%,combined with high frequency color Doppler ultrasound detection rate was 86.5%,of which the was not detected in patients with MRI,the detection rate was 100%.Total effective rate of Anal fistula in group B was100.00%,it was significantly higher than group A of 76.92%,(Zc=2.023?P=0.043).There was no significances(P<0.05)between the two groups in the aspects of postoperative pain score(t=3.578 ? P=0.001)? postoperative healing time(t=14.201 ? P=0.000)?postoperative healing time(t=3.782?P=0.000)and recurrence rate(x2=4.127?P=0.042).However,there were no statistical significances(P > 0.05)between the two groups inanal sphincter fistula cut score(t=1.694?P=0.097)and quality of life score(t=1.870?P=0.067).Conclusion Primary tuberculous anal fistula with pulmonary tuberculosis,should be diagnosed based on pathological examination or anal secretions smear andculture results as the gold standard,before and after anti tuberculosis treatment of the timing of surgery and prognosis of patients have a certain relationship,the first antituberculosis treatment after surgery may help to improve the prognosis of patients.
