Objective:To study the efficacy of simple and comprehensive treatment of early postoperative clamped drainage combined with cryotherapy minimizing blood loss after total hip arthroplasty.Methods:A total of 60 patients with total hip arthroplasty were randomly selected from September 2015 to October 2016 at the Second Affiliated Hospital of Jilin University.The patients were randomly divided into three groups.The control group(postoperative unclamped drainage and was not treated with cryotherapy),the cryotherapy group(postoperative unclamped drainage and was treated with cryotherapy for 24 hours),the comprehensive group(early postoperative clamped drainage for 4 hours and was treated with cryotherapy for 24 hours).There were 20 patients in each groupes,There were 20 patients in the control group,including 10 males,10 females,8 patients due to the femoral neck fracture,12 patients due to the femoral head necrosis,13 patients in the right side of the limb,7 patients in the left,the average age of these patients was 62.80 years old with an average body weight of 65.85 kg and an average body mass indexof 23.92.There were 20 patients in the cryotherapy group,including 10 males,10 females,6 patients due to the femoral neck fracture,14 patients due to femoral head necrosis,The average body weight was 69.05 kg and the mean body mass index was 26.24.There were 20 patients in the comprehensive group,including 7 males,13 females,7 patients due to femoral neck fracture,13 patients due to femoral head necrosis.There were 7 cases on the right side of the limb and 13 cases on the left side.The mean age was 63.05 years,the average body weight was 61.25 kg and the average body mass index was 23.61.The postoperative blood loss,drainage volume,invisible blood loss and postoperative complications were observed to assess the impact of blood loss and the recent safety of the comprehensive measure of early clamped drainage combined with cryotherapy after the total hip arthroplasty.Result:Compared with the control group,the total blood loss of the cryotherapy group was less(P <0.05),the amount of invisible blood loss was less(P <0.05),the drainage volume was similar significantly(P>0.05),the postoperative complications were not statistically significant.Compared with the cryotherapy group,the total blood loss and drainage volume of the comprehensive group(P <0.05)was much less,and the amount of invisible blood loss was similar significantly(P> 0.05).There was no significant difference between the two groups inpostoperative complications.Conclusion:For the patients who receive first total hip arthroplasty,24 hours of continuous cryotherapy can reduce postoperative invisible blood loss,reducing the total blood loss after surgery,combination with early clamped drainage on this basis can reduce the drainage volume without increasing invisible blood loss,further reducing postoperative total blood loss,at the same time the incidence of postoperative hematoma,infection and deep vein thrombosis was not increased.It is simple and easy treatment for the patients who receive first total hip arthroplasty that early clamped drainage combined with cryotherapy comprehensive measures can be more effective to reduce postoperative blood loss.
