Research purpose: the research task mainly analyzes syndrome differentiation and treatment rule of purulent otitis media(POM),bases on consulting and research of the ancient and modern literature,as well as collection and analysis of data,summarizes the syndrome differentiation and treatment rule of ancient and modern physicians in POM,proposes the syndrome differentiation and treatment method of treating POM,hopes to provide reference for modern medical research and clinical treatment of POM.Research method: this issue determines the selected literature through various retrieval modes,and concludes the literature arrangement,through comparative analysis,make a comparison of the treatment rule and pharmacy of POM,confirm the similarities and differences between the ancient and modern treatment of POM,and put forward personal opinion for reference.The source of research object: ancient literature mainly refers to various times of physician works,searches records and related literature about POM,and reads over summary,determines the selected literature.Modern literature mainly selects the relevant Chinese medicine and western medicine literature about POM published in domestic academic journal from 1990 to 2016.The inclusion criteria of literature: the content of article is related to POM treatment.The article-published unit belongs to the formal legal periodical of the National News Publication Bureau,time is from 1990 to 2016.The patients are clearly diagnosed as POM,the case amount reported in this article is at least 10.The patients with clear syndrome differentiation treatment are preferred.Prescription inclusion criteria: the pharmacy for traditional Chinese medicine or Chinese-western combined medicine.The prescription has a clear composition,treatment effect gains recognition.Be elected by external treatment and take-in medicine.Prescription exclusion criteria: only for western medicine treatment.Prescription composition is not clear.The establishment of database:use Excel software to establish the research database.The content mainly includes: etiology and pathogenesis,therapy,prescription,etc.Use Excel software to analyze the contents to be analyzed,and acquire the results.Research result:(1).The inclusive research literature: find out 52 rolls of contents related to etiology and pathogenesis of POM and its treatment pharmacy from 45 physicians works in the ancient literature.As for modern literature,through VIP,How Net and other databases retrieval,search 430 related literature about POM in total,and then conduct advanced search of keywords "otitis media suppurative","traditional Chinese medicine" "POM”and so on,read over and summary each article,exclude the literature with inconformity to research standard,61 literature conforming to standard are screened.(2).Analysis of etiology and pathogenesis: in ancient literature,discuss the relationship between POM and viscera,which mainly can be divided into: 23 articles related to kidney,among which 12 articles about thermojunction of kidney,7 articles about kidney qi deficiency,4 articles about kidney-yin deficiency.11 articles about liver:8 articles about liver and gall fire hyperactivity,3 articles about liver wind hyperactivity.And 7 articles about heart:1 article about heart fire hyperactivity,6articles about wind-heat toxin blocking heart.And 4 articles about spleen: 4 articles related to insufficiency of the spleen and damp stomach.1 article about lung: 1 article about lung qi deficiency.The 61 modern articles are mainly related to the discussion of the lung,liver,spleen,kidney.The treatment can be divided into: 36 articles about lung,36 articles about lung channel wind-heat.35 articles about lung,26 articles about liver and gall fire hyperactivity,9 articles about wind-heat invasion with the liver channel and ear.21 articles about kidney,21 articles about kidney loss.27 articles about spleen: 27 articles about weakness of the spleen and the stomach.(3).The analysis of treatment based on differentiation of symptoms.Ancient physicians mainly divide POM into six types: syndrome of kidney Qi thermojunction,liver and gall fire hyperactivity syndrome,kidney loss syndrome,wind-heat invasion syndrome,insufficiency of the spleen and stomach dampness syndrome,liver hyperactivity syndrome.Modern physicians mainly divide POM into four types syndrome: wind heat invasion syndrome,spleen and stomach deficiency syndrome,liver and gall fire hyperactivity syndrome,kidney deficiency syndrome.(4).Analysis of drug use: ancient medicine for external use mainly includes medicine for insecticide and detoxification,eliminating dampness nd antipruritic(alumen instead of aluminite),awakening drugs(musk,borneol),detoxification and reproduction of muscle drug(lead powder,calomel),invigorating blood circulation and eliminating stadis prescription(dry Rouge).Ancient take-in medicine mainly includes heat-clearing drug(rehmannia,gardenia,Scutellaria baicalensis,root of common peony,moutan bark,forsythia suspensa,wild chrysanthemum,Radix trichosanthis,Radix Scrophulariae),drugs for relieving exterior syndrome(bupleurum,radices sileris,Cimicifugae foetidae,fructus viticis,Schizonepeta,Arctium),qi-tonifying drug(glycyrrhiza uralensis,ginseng,Chinese yam,Astragali radix),damp-clearing drug(poria cocos,Akebiaquinata,Alisma Plantago,plantago asiatica).The highest-frequency appearance of modern external-use medicines are heat-clearing drugs(Coptis,phellodendron,Radix Sophorae Flavescentis,Radix Scutellariae),awakening drugs(borneol,musk),drug for insecticide,detoxification,eliminating dampness and antipruritic(alumen).The highest-frequency appearance of modern take-in medicine are heat-clearing drug(honeysuckle,dandelion,Scutellaria,phellodendron,wild chrysanthemum,Radix Scrophulariae,forsythia),qi-tonifying drug(glycyrrhiza uralensis,astragalus membranaceus,Atractylodes,Codonopsis,Chinese yam),damp-clearing drug(Poria cocos,coix seed,Alisma,Plantago),drugs for relieving exterior syndrome(Angelica,bupleurum,radices sileris,Cimicifuga).Discussion: through the study and summary of ancient and modern literature related to POM,this paper proposes discussion opinion on the etiology andpathogenesis of POM,discrimination thought,syndrome differentiation and treatment.I think the treatment of POM based on syndrome differentiation and surgical treatment of skin and eternal diseases have the same purpose.The incidence of POM can be divided into three periods-preterm,medium term and late term,the preterm adopts “elimination” method,the middle term adopts “support” method,the late term adopts the "fill" method,the three methods flexibly are employed in each period of treatment.The main syndromes of pre-period are wind-heat lung channel syndrome,Shaoyang windheat syndrome,gall and liver fire hyperactivity syndrome;the main syndromes of the medium-term are spleen deficiency and dampness stagnation syndrome;the main syndromes of the late-term are kidney yin deficiency syndrome and kidney yang deficiency syndrome.
