ObjectiveTo evaluate the accuracy and limitation of rapid frozen diagnosis to squamous intraepithelial lesion.To discuss the value of visual inspection with lugol's iodine in frozen diagnosis of squamous intraepithelial lesion when analyzing rapid frozen section examination.MethodTotally 384 cases of patients with conization of cervix disease who were in frozen section examination(FSE)in the First Hospital affiliated of Zhengzhou University from March 2016 to July 2016 were collected.Finally 294 cases were screened out for retrospective analysis.The patients aged from 18 to 90 years,with average age by 43.54 years.Wherein 150 cases were not for VILI experiment;the other 144 cases were for VILI experiment.If it is confirmed to be suspicious diagnosis after conventional diagnosis,immunohistochemical diagnosis will be re-implemented to get ultimate conventional diagnosis.All the included patients for research shall be in accordance with the inclusive criteria for indications of conization of cervix,and sign the informed consent.The SPSS17.0 software was used to compare various statistic data of FSE diagnosis and ultimate PSE diagnosis,such as positive predictive value(+PV),negative predictive value(-PV),sensitivity(Se),specificity(Sp),positive likelihood ratio(+LR),negative likelihood ratio(-LR),Youden's index,diagnostic odds ratio(DOR)and coincidence rate.And the aforementioned data were statistically analyzed,with percentage rage representing counting data,?2 test was used to statistically analyze the counting data,with ?=0.05 used as test level.ResultsAs for VILI group,Se,Sp,+PV,-PV,+LR,-LR,DOR,Youden's index and coincidence rate were 94.12%,72.73%,77.53%,92.52%,3.45,0.08,2.67,0.67 and 77.78% respectively for diagnosis of FSE cervix inflammatory lesion;Se,Sp,+PV,-PV,+LR,-LR,DOR,Youden's index and coincidence rate were 57.14%,89.91%,84.99%,67.72%,5.66,0.48,11.88,0.47 and 81.94% respectively for diagnosis of FSE low-grade squamous intraepithelial lesion;Se,Sp,+PV,-PV,+LR,-LR,DOR,Youden's index and coincidence rate were 66.67%,98.55%,97.87%,74.73%,46.00,0.34,136.00,0.65 and 81.94% respectively for diagnosis of FSE high-grade squamous intraepithelial lesion.As for non-VILI group,Se,Sp,+PV,-PV,+LR,-LR,DOR,Youden's index and coincidence rate were 96.77%,58.82%,70.15%,94.80%,2.35,0.05,42.86,0.56 and 66.66% respectively for diagnosis of FSE cervix inflammatory lesion;Se,Sp,+PV,-PV,+LR,-LR,DOR,Youden's index and coincidence rate were 35.62%,89.51%,74.72%,57.79%,2.96,0.65,4.45,0.24 and 80.00% respectively for diagnosis of FSE low-grade squamous intraepithelial lesion;Se,Sp,+PV,-PV,+LR,-LR,DOR,Youden's index and coincidence rate were 51.62%,98.25%,96.71%,67.00%,29.42,0.49,59.73,0.49 and 69.33% respectively for diagnosis of FSE high-grade squamous intraepithelial lesion.After ?2 test,the accuracy for VILI group(70.83%)were higher than that for non-VILI group(58%).The diagnosis accuracy for the two groups of experiments had difference which had statistic significance(P<0.05).ConclusionVILI benefits to improve the diagnosis accuracy of frozen section examination to squamous intraepithelial lesion.
