| Objective:Cervical cold knife cut method(CKC)cone to at present has become the hot spot of the influence of fertility,this study analysis under different influencing factors of high level cervical squamous intraepithelial lesion(HSIL)CKC postoperative patients with pregnancy and pregnancy outcome of relationships,investigate the cervical cold knife cut method of the cone resection range on pregnancy and preterm premature rupture of membranes,impact,To investigate the effect of cervical conectomy on pregnancy,premature delivery and premature rupture of membranes,and to provide clinical guidance for cervical conectomy and repregnancy in HSIL patients with fertility requirements.Methods:A total of 176 patients aged 18-30 who received CKC treatment for HSIL in The Affiliated Hospital XXX,1,2016 to December 31,2019 were selected for this study.The age,body mass index(BMI),pregnancy and delivery times,HPV infection type and cervical erosion change degree of patients at the time of CKC were collected.Conical CKC resection range(conical resection height and conical resection volume),pathological results before and after surgery,pregnancy after CKC,pregnancy time,pregnancy mode,preterm labor(PTD),PROM,and pregnancy duration were divided into pregnancy group and non-pregnancy group.Empower Stats and R software were used for statistical analysis.Under different influencing factors,the range of CKC resection(height and volume)and the risk of premature delivery and premature rupture of membranes as well as the related factors affecting pregnancy between the two groups were analyzed.When P<0.05(bilateral),it was statistically significant.Results:1.There was no significant difference between pregnant group and non-pregnant group in conical resection height,conical resection volume and other factors such as age,BMI,pregnancy and delivery,HPV infection type,etc.2.There was no correlation between age,BMI,gestational times,degree of cervical erosion,vaginitis,HPV infection type,fasting blood glucose,,conical resection volume,postoperative pregnancy time and premature delivery,premature rupture of membranes and low birth weight.(P values were greater than 0.05).3.The relationship between the height of CKC and preterm birth in HSIL patients was statistically significant(P=0.047).Each 1cm increase in CKC height was associated with a 14.2-fold increased risk of preterm birth,and each lcm increase in CKC height was associated with a 14.8-fold increased risk of preterm birth in patients infected with HPV16(P=0.043).The preterm birth rate increased significantly when cervical conical height was greater than 2.0cm.Conclusion:High CKC in HSIL was significantly associated with preterm birth,and the risk of preterm birth was even more significant in patients infected with HPV type 16. |
