Reconstruction Of Acetabular Rotation Center And Functional Recovery After Total Hip Arthroplasty

Background: With the rapid development of economy,increase the change in people's way of life and the degree of population aging,osteonecrosis,osteoarthritis of the hip and femoral neck fracture and hip joint disease incidence increased year by year.Total hip arthroplasty can cause joint amputated patients' pain and activity limitation,replace diseased hip artificial joint wear,reduce the patient's pain is good,the reconstruction of the hip joint function,highly praised by the majority of clinicians.It is very important to reconstruct the rotation center of the acetabulum in the successful total hip arthroplasty,and more and more attention has been paid to the total hip arthroplasty.In this paper,the effect of reconstruction of rotation center on the recovery of hip function after total hip arthroplasty in patients with cementlessprosthesis is discussed.Methods: This study is a retrospective study,the research objects were patients of joint surgery in the Second Affiliated Hospital of Dalian Medical University.All patients received unilateral total hip replacement in the hospital,all the patients are from January 2014 to January 2016 in our hospital patients,a total of 87 patients were included in this topic among them,72 patients suffering from severe necrosis of the femoral head,and 15 patients with severe osteoarthritis of the hip,offset this topic mainly want to discuss after total hip arthroplasty rotation center had a small effect on the postoperative functional recovery of hip joint,so all patients according to the postoperative acetabular rotation center whether or not the reconstruction of acetabular reconstruction is divided into group A(62 cases)and B group without reconstruction of acetabular(25 cases).All patients had severe pain with varying degrees of activity limitation,quality of life were significantly decreased,there are clear indications of total hip arthroplasty.All patients had no obvious contraindications,risk to patients and their families account operation and signed consent form.By our hospital joint surgery total hip arthroplasty experienced chief surgeon surgery,all patients prosthesis were non bone cement prosthesis,for total hip arthroplasty.Before and after surgery were taken standard radiograph of the pelvis to determine whether the rotation center(the center of rotation in the anatomical reconstruction of prosthesis rotation center in 5mm that is regarded as the reconstruction),the patients were followed up for 6~30 months,an average of 18 months.Patients with hip joint function recovery were assessed by Harris score before and at the last follow-up.The Harris hip function score of two groups were compared before and at the last follow-up,and the data were analyzed by SPSS software.Results: After the analysis of SPSS software,at the end of the follow-up Harris hip score of the two groups were significantly improved,and the reconstruction of the acetabular rotation center than those without the reconstruction were higher,and the difference was statistically significant.Conclusion: Severe pain with activity limitation of patients with femoral head necrosis and hip osteoarthritis patients after total hip arthroplasty,the patient's pain has been greatly reduced or completely disappeared,hip flexion rotation in each direction of the scope of activities has been significantly improved,after a period of rehabilitation walking gait is close to even reach the normal level,the quality of life of the patients improved significantly.Therefore,total hip arthroplasty is an effective surgical method,is the hip in patients with advanced lesions of the gospel.Total hip arthroplasty success should include operation of sufficient preparation before operation and precision,the rotation center of hip joint reconstruction accurately is important for the success of total hip arthroplasty,because anatomical reconstruction of the center of rotation can produce smaller joint stress,is conducive to the recovery of the mechanical environment around the hip joint.You can get a stable hip joint,so after the same rehabilitation training can get better hip function improvement of patients satisfaction is higher,so the reconstruction of the center of rotation in the total hip arthroplasty plays a more and more important position,if other factors remain unchanged,the functional recovery of hip joint rotation from the center of anatomical reconstruction obtained great benefits.So how to the center of rotation for accurate reconstruction,in order to achieve this goal,need a doctor before operation to the center of rotation for precise positioning and operation plan as detailed as possible,improve intraoperative careful operation and careful positioning.Because this study is a retrospective study,and the sample size is small,there is a large gap between the two groups,the persuasion is not as good as the prospective large sample test,and the follow-up time is relatively short.The lack of long-term data such as: late hip function,artificial hip wear rate,loosening rate,prosthesis life data were not included in this trial.Only the early joint function was evaluated,follow-up work required longer follow-up and more patients for further clinical study.
