Objective:Use of hip rotation center position, for total hip replacement the guiding significance of double lower limbs length difference effect, accurate positioning respectively using special tool prosthesis the acetabulum, femoral head rotation center axial location, combined with preoperative X-ray template to measure the contralateral acetabulum and femoral head rotation center position, discussed to reduce total hip replacement surgery a new method of double lower limbs length difference.Methods:In September2012to December2013,50patients with primary total hip replacement,(20male and female30cases;30-82years old (average age59.38years);Left hip23cases,27cases of right hip,Of20patients with avascular necrosis of the femoral neck fractures in10cases,10cases of hip arthritis, developmental dysplasia of the hip9cases,1case of rheumatoid arthritis.Preoperative X-ray template measurement, reference before the contralateral the acetabulum, femoral head rotation center location, use special tools to guide the intraoperative side implantation of acetabulum and femoral head prosthesis position. After measuring the length of lower limbs.Results:Intraoperative compared with preoperative measurement,2patients length difference is2mm,6patients length difference is1mm, the other no difference was seen in patients with preoperative measurement, according to the preoperative design, according to the formula of the contralateral acetabulum center height-femoral head height=of acetabulum center height-femoral head height, placement prosthesis, postoperative double lower limbs length variation from0to5mm, postoperative range on average length is0.17cm.Conclusions:Through our attention of double lower limbs length difference and exploration, we can get the conclusion that by preoperative X-ray measurement, combined with preoperative X-ray template to measure the contralateral the acetabulum, femoral head rotation center location, use special tools to pinpoint prosthesis the acetabulum, femoral head rotation center axial location, appropriate adjustments, double lower limbs length difference can be obtained to minimize, reach the state of the patient satisfaction by reducing total hip replacement surgery a new method of double lower limbs length difference.
