Observation On Effect Of Homemade Hot Compress Cuff For The Complications Of The Surgery Patients With Venous Indwelling Needles

ObjectivesDevelop a hot compress cuff,through the application of indwelling needle in older surgical patients,observation on effect of homemade hot compress cuff for the older women patients with venous indwelling needles,explore the prevention measures of the complications of older surgery patients with venous indwelling needled and improve the comfort of the patients.MethodsFrom November 2015 to October 2016,120 older surgical patients in the gynaecology ward were recruited and divided into the experimental group and control group,60 cases in each group.Both groups were placed venous indwelling needles in the operating room.Control group back to the ward from the operating room,the infusion side of the body without hot compress.Back to the ward,the experimental group patients use homemade hot compress cuff for the infusion side of the body heat,until the indwelling needle removal.Hot parts for the forearm of the infusion side,the temperature is set to 40 degrees,the once heat for 30 minutes,heat once every two hours,until infusion is completed.We measured the skintemperature with Mike doctor non-contact infrared thermometer on back to the ward and after the infusion,before and after the infusion on the first day after surgery,before and after the infusion on the second day after surgery.In addition to skin temperature,record the phlebitis,obstruction,indwelling time,comfort and pain scores.The SPSS 21.0 statistical software was used for data statistical analysis,and the test level is 0.05,P<0.05 indicated the difference was statistically significant.In the General data,the quantitative data were analyzed by independent-sample t test and Mann-Whitney U test,qualitative data by chi-square test.The changes of forearm skin temperature of infusion side were analyzed by independent-samples t test,the non-normal distribution data were analyzed by Mann-Whitney U test,the changes of forearm skin temperature of both sides in the group were analyzed by paired t test and the non-normal distribution data were analyzed by Wilcoxon rank sum test.The phlebitis,obstruction were analyzed by chi-square test and Kruskal Wallis test,indwelling time,comfort and pain assessment were analyzed by Mann-Whitney U test.Results1.General information:there were no statistically significant differences in age,body mass index,information related to the operation,et al between the two groups(P>0.05).2.The changes of skin temperature of two groups:(1)Compared the skin temperature of infusion side arm of two groups,the skin temperature of the experimental groups were increased within three days,respectively(2.70 ±0.65)℃,(0.53±0.48)℃ and(0.35±0.39)℃,the control group was increased(1.57±0.82)℃ in the first day,the next two days were decreased,the temperature was decreased(0.23±0.51)℃ and(0.09±0.35)℃,respectively.The temperature of the experimental group was higher than that of the control group,and the difference was statistically significant(P <0.05).(2)Compared the skin temperature of the both sides of the experimental group,the temperature of infusion side were increased withinthree days,respectively(2.70±0.65)℃,(0.53±0.48)℃ and(0.35±0.39)℃,the other side were increased respectively(2.08±0.63)℃,(0.36±0.54)℃ and(0.19±0.34)℃,and the difference was statistically significant(P <0.05).(3)Compared the skin temperature of the both sides of the control group,in the first day the temperature of infusion side was increased(1.57 ± 0.82)℃,the other side was increased(1.97 ±0.82)℃,the difference was statistically significant(P <0.05).The next two days,the temperatures of the infusion side were decreased,respectively(0.23±0.51)℃ and(0.09±0.35)℃,the other side were increased,respectively(0.29±0.64)℃ and(0.16± 0.45)℃,the temperature of infusion side were lower than the other side,respectively(0.52±0.86)℃ and(0.25±0.48)℃,and the difference was statistically significant(P <0.05).3.There were 4 cases of phlebitis in the experimental group,and all is grade 1.The control group occurred phlebitis 12 cases,including grade 1 of phlebitis 9 cases,grade 2 of phlebitis 3 cases.The incidence of phlebitis in the experimental groups(6.7%)was significantly lower than that in the control group(20.6%),and the difference was statistically significant(c2=4.794,P <0.05),compared the degree of phlebitis between the two groups are statistically significant(H=4.979,P<0.05).4.In the first and second days after operation,compared the blocking rate of the two groups,the difference were not statistically significant(P> 0.05).5.The indwelling time of the experimental group(53.60±4.67)h was longer than that of the control group(50.43 ± 7.96)h,there was a statistically significant differences(P<0.05).Compared the removal time of two groups,there was no statistically significant differences(P> 0.05).6.The comfort score of the control group was(0.31±0.46),the experimental group was(0.12 ± 0.32),compared with the experimental group,there was a statistically significant differences(Z=-2.518,P<0.05);the pain score of the control group was(2.10±1.92),the experimental group was(0.81±1.18),compared the pain score of two groups,there was a statistically significant differences(Z=-3.823,P<0.05).Conclusions1.After the operation,the infusion side of the body using homemade hot compress cuff for local heat,to a certain extent can promote the recovery of limb temperature,keep the limb to warm,reduce the cold feeling of limb.2.Used the hot compress cuff to older surgical patients with indwelling needle for local heat can reduce the rate of phlebitis,improve the effect of tube plugging need further research.3.The homemade hot compress cuff can improve the comfort of the patients during the infusion period,reduce the pain caused by drug.
