Objective: To study the clinical efficacy of budesonide and formoterol fumarate inhalation combined with dust mite allergen desensitization therapy for moderate to severe bronchial asthma.Method: Eighty moderate to severe bronchial asthma patients who were sensitive to dust mite were divided into control group and treatment group.Forty patients were treated with budesonide and formoterol fumarate inhalation agent(160/4.5 μg,2 puffs per day,morning and evening,approximately 12 hours apart)lasting for 12 months as control group.The other forty patients were treated besides budesonide and formoterol fumarate inhalation agent as above and combined with dust mite allergen desensitization therapy lasting for 12 months as treatment group.For desensitization therapy of treatment group,the vaccine was injected in the subcutaneous part of deltoid muscle.The initial minimum dose 1/100000 one time per week,dose was increased according to 0.1,0.2,0.4,0.6,0.8 ml,when the amount of maximum dose was maintained at 0.8 ml(concentration was 1/5000).It was injected once in every 4 weeks lasting for 12 months.The evaluation of asthma symptoms,lung function and quality of life(AQLQ)before and after treatment for 3,6,12 months were observed respectively.Results: The asthma symptoms score at day and night were significantly lower thanthose before treatment in control group and treatment group;Also in the two groups,the forced expiratory volume in one second(FEVl)and peak expiratory flow(PEF)after 3,6,12 months treatment were significantly higher than those before treatment(P <0.05).The AQLQ score(the limitation of activity,the symptoms of asthma,the psychological state and health care)was significantly higher in the 12 months after the treatment(P<0.05).Furthermore,compare with the control group,the asthma symptoms score at day and night after 12 months treatment was more lower(P<0.05),the AQLQ score was more significantly higher(P<0.05),and FEVl and PEF improvement were more significantly higher in the treatment group(P<0.05).Also,the average time of asthma control,the number of acute asthma attack,the number of emergency drug use in treatment group was significantly lower than those in control group(P<0.05).The incidence of adverse reactions in the treatment group was 15.98% and no serious adverse reactions occurred.Conclusion: Budesonide and formoterol fumarate inhalation combined with dust mite allergen desensitization therapy in the treatment of moderate to severe bronchial asthma can improve the symptoms of asthma and lung function.The desensitization therapy is beneficial to improve the symptoms and quality of life for a long time without server adverse reactions.
