ObjectiveTo observe the changes of urinary NGAL,?2-MG,NAG,blood glucose,blood lipids and the changes of IL-6 and oxidative stress markers in the urine of patients with early diabetic nephropathy before and after the method of invigorating the spleen and kidney,promoting blood circulation.To explore the possible mechanism of the method of invigorating the spleen and kidney,promoting blood circulation on renal tubular protection in patients with early diabetic nephropathy.method60 patients with early diabetic nephropathy were randomly divided into control group and treatment group.The control group was treated with basic treatment-The treatment group was treated with the method of invigorating the spleen and kidney,promoting blood circulation on the basis of the control group.After 4 weeks,detect the levels of urine NGAL,urinary ?2-MG,urinary NAG,serum creatinine,endogenous creatinine clearance,blood glucose,blood lipid and other biochemical indexes,inflammatory markers IL-6 and oxidative stress markers SOD.results(1)Urinary NGAL,urinary ?2-MG and urinary NAG were compared between the two groups before and after treatment:Urinary NGAL,?2-MG and NAG decreased after treatment,the difference was statistically significant(P<0.05).Compared with the control group,the levels of NGAL,?2-MG and NAG in the treatment group were lower than those in the control group(P<0.05)(2)Blood glucose,blood lipid and blood pressure of the two groups before and after treatment:Compared with the treatment group,the blood glucose,blood lipid and blood pressure were decreased after treatment,the difference was statistically significant(P<0.05),the control group after treatment,blood pressure,The difference was statistically significant(P<0.05),blood glucose,blood lipid slightly lower,but the difference was not statistically significant.Compared with the control group,the diastolic blood pressure of the treatment group was significantly lower than that of the control group(P<0.05).The difference was statistically significant(P<0.05),and the difference was statistically significant(P<0.05).After treatment,the systolic blood pressure of the treatment group was slightly lower than that of the control group,but the difference was not statistically significant(P>0.05).(3)The levels of IL-6 and oxidative stress in the two groups were significantly higher than those before treatment(P<0.05).Compared with before treatment,the levels of IL-6 in the two groups were lower than those before treatment,and the difference was statistically significant(P<0.05).Compared with the control group,the levels of IL-6 in the treatment group were lower than those in the control group,and the difference was statistically significant(P<0.05)(4)Before and after treatment,the levels of serum creatinine and endogenous creatinine in the two groups were significantly lower than those before treatment(P<0.05).Compared with the two groups before and after treatment,The serum creatinine and endogenous creatinine clearance rate in the treatment group were lower than those in the control group(P<0.05)(5)Correlation analysis:urine NGAL and other renal injury markers urine NAG,urine ?2-MG,urinary microalbumin was positively correlated,r values were 0.418,0.360,0.404(P<0.05);urinary NGAL and blood glucose(P<0.05).Urine NGA1 was positively correlated with IL-6,and r value was 0.523(P<0.05).The positive correlation between urinary NGA1 and IL-6 was 0.35,0.05).Urine NGAL was negatively correlated with oxidative stress markers,r value was-0.639(P<0.05).Urine NGAL was positively correlated with serum creatinine and endogenous creatinine clearance rate,r value was 0.471,0.529(P<0.05).conclusion(1)The method of invigorating the spleen and kidney,promoting blood circulation can reduce urinary NGAL levels in patients with early diabetic nephropathy,effectively protect the renal tubules in patients with early diabetic nephropathy,delay the progress of diabetic nephropathy.(2)The method of invigorating the spleen and kidney,promoting blood circulation can reduce the level of IL-6 in patients with early diabetic nephropathy and improve the level of SOD in patients with early diabetic nephropathy,which can effectively improve the microinflammatory state and oxidative stress in early diabetic nephropathy,and have anti-Oxidative stress.(3)The method of invigorating the spleen and kidney,promoting blood circulation can effectively regulate early diabetic nephropathy in patients with blood sugar,blood lipid levels.(4)The method of invigorating the spleen and kidney,promoting blood circulation method can effectively regulate the blood pressure levels of patients with early diabetic nephropathy,reduce glomerular filtration pressure,reduce renal perfusion.(5)Urinary NGAL can be associated with urinary ?2-MG,urinary NAG,urinary microalbumin together to become early diabetic nephropathy markers.(6)The method of invigorating the spleen and kidney,promoting blood circulation to protect the early diabetic nephropathy in patients with renal
