ObjectiveThe purpose of this study was to evaluate the clinical application and safety of MRI-conditional Cardiovascular Implantable Electronic Device.MethodsA retrospective review of all patients with MRI-conditional CIED admitted in cardiovascular department of the second affiliated hospital of medical school,Zhejiang University from June 2014 to November 2016 was conducted.The baseline characteristics of the study patients,information of Implanted Cardiac Devices,state of MRI examination performed at 1.5 Tesla,follow-up data which includes complications as well as lead parameters(sensing threshold,pacing threshold,lead impedance)assessed during and 48hrs after operation,after 1 month,3 months and 6 months were collected and analyzed to investigated the use of the MR conditional CIED in current days.Results437 patients(male 246[56.1%],age 69.1 ± 12.9 years,Pacemaker replacement 2[0.5%],PM 418[95.6%],ICD 15[3.4%],CRT 2[0.5%])were enrolled.Lead dislodgement within 30 days of pacemaker implantation occurred in 2 patients.24 patients underwent MRI during the observation period.No MR related SADE occurred.Lead measurements showed an increse in sensing threshold as well as a decrese in lead impedance between during(RA/RV sensing threshold 3.1 ± 1.5mV/10.8 ± 6.3mV,lead impedance 624 ± 149Ω/740 ± 165Ω)and 48hrs after operation(RA/RV sensing 3.5 ±1.9mV/13.2 ± 5.3mV,lead impedance 517 ± 85 Ω/602 ± 105 Ω).There was no significant change of pacing capture threshold during(RA/RV 1.03 ± 0.68V/0.76 ±0.63V)and 48hrs after operation(RA/RV 0.79 ± 0.27V/0.78 ± 0.42V)(p>0.05).Lead measurements at 48hrs after operation and 1 month to 6 months after operation were not statistically different.ConclusionsThis supports the evidence that patients with MR conditional CIED can safely undergo MRI in a 1.5T system.Short term follow-up did not reveal significant changes in pacing capture nor sensing threshold.The use of the MR conditional CIED is feasible for MRI-required patients.
