| ObjectiveThe status of-pediatric medical service ability of maternal and child health hospital in district level in Guangzhou city was investigated.The evaluation index system of pediatric medical service ability of maternal and child health hospital in district level in Guangzhou city was constructed.The problems of pediatric medical service ability of maternal and child health hospital in district level in Guangzhou city was analyzed,basing on the survey.The suggestion about promoting pediatric medical service ability of maternal and child health hospital in district level in Guangzhou city was put forward,which provided the reference for decision-making of the health administration departments.MethodsThe evaluation index system of pediatric medical service ability of maternal and child health hospital in district level in Guangzhou city was constructed,by the methods of Literature research,expert interview,Delphi.AHP was used to calculate the weight of indicators at all levels.Questionnaire survey was used to collect the data of pediatric medical service ability of maternal and child health hospital.Method of topsis was used to evaluate 4 maternal and child health care centers in Guangzhou.ResultsFirstly,the overall condition of pediatric in Guangzhou medical institution was introduced.There were 110 hospitals with pediatric department and the doctor-to-nurse ratio was 1:1.26.Highly educated pediatricians with senior positional titles were concentrated on Tertiary Hospital.Pediatricians get lower income and more workload than average level of the overall hospital.Secondly,pediatric medical service situation was showed.The number of pediatricians declined from 147 in 2013 to 140 in 2015,while the nurses increased from 272 in 2013 to 281 in 2014 and then descended to 271 in 2015.Take the year of 2015 as an-example,the proportion of medical staff with senior positional title and master degree were 20%and 8%,and the proportion of nurses with intermediate title,bachelor or master degree were 10%,22%and 0.From 2013 to 2015,the average length of stay fell off and rearing bed ascended while ward turnover frequency descended in district level Maternal and Child Health Hospital of Guangzhou.Thirdly,Delphi method was used to build the evaluation system,including 4 first-class index of human resource,material,finance and medical service,10 second-class index of staff number,structures and number of beds,24 third-class index of total number of medical staff,pediatrician per thousand children,doctor-to-nurse ratio and pediatrician with bachelor degree or above.Fourthly,AHP method was used to measure the index weight based on the evaluation system.The top three of index weight were human.resource(0.4550),structures(0.3640)and finance(0.2627).Ranking by TOPSIS method,the hospital with best medical service among the four district level Maternal and Child Health Hospitals was H3 and the Ci value of which was 0.7046,while the worst one-was Hl and Ci value of which was 0.1768.ConclusionsFirstly,The pediatric medical staff,of which the qualifications should be enhanced,was in a small amount.They were-under a lot of strain at work,thus the staff turnover was frequent.The beds resources were not enough to meet the demand of patients.The pain and gain of the pediatric medical staff are were not balanced.The financial compensation for pediatric department was inadequate.Secondly,the evaluation system for pediatric medical service ability of maternal and child health hospital in district level in Guangzhou city was established and AHP was used to measure the index weight.The pediatric medical service ability of the four maternal and child health hospital are ranked by TOPSIS,which also tells the operability of the evaluation system.Thirdly,strategies were given to several aspects such as government,hospital,medical staff and patients in order to improve the service Ability of pediatric medical service ability of maternal and child health hospital in district level in Guangzhou. |
