The Clinical Research Of Lumbar Disc Herniation Of Xubi Type Treated In Transforaminal Selective Nerve Root Block And Xubi Prescription

ObjectiveThe transforaminal selective nerve root block(Transforaminal selective nerve root block)was the use of target puncture technology to solve the disease directly to the lesion;Xubi prescription was Self-designed clinical experience prescription which Professor Lin Yifeng based on the theory of"Wenyang Du".Based on the clinical symptoms,laboratory indexes such as observation,objective evaluation the clinical curative effect of two kinds of treatment,which provided the best treatment for clinical treatment of lumbar intervertebral disc protrusion.MethodsThe 72 patients accorded with the condtions which were selected from the third affiliated hospital of guangzhou university of TCM spinal orthopaedic from the February 2015 to February 2016,who were were randomly divided into two groups on the basis of signing the "Informed Consent of Clinical Research",which included test group A who received the transforaminal selective nerve root block and the clinical pathway of the spinal orthopaedic and test group B which received the Xubi prescription and the clinical pathway of the spinal orthopaedic.Following up 12 months postoperative,4 cases were droped out of the study and 3 cases were removed(3 cases in group A and 4 cases in group B).65 patients(33 patients in group A and 32 patients in group B)could finish the study.concentration of IL-6 in serum were compared during preoperative and postoperative 1 day,1 month,6 months and 12 months,and the pain VAS score and low back pain-JOA score were compared during preoperative and postoperative 1 day,1 month,3months,6 months and 12 months.The total fine rate were compared at 12 months postoperatively.Which had an objective evaluation of two kinds of treatment,so as to find more rapid,effective treatment of the lumbar disc herniation.ResultsThere was no significant difference between the two groups in general data(gender,age,age distribution interval,duration of disease and responsibility period),which was not statistically significant(P>0.05).The pain VAS score and low back pain-JOA score,concentration of IL-6 in serum by the group of transforaminal selective nerve root block improved significantly after Postoperative day 1,the group of Xubi prescription did not have the obvious improvement;after Postoperative 1 month and 3 months two groups of patients with pain VAS score,low back pain JOA score and concentration of IL-6 after Postoperative 1 month in serum had more significant difference(P<0.05),the effect of group of transforaminal selective nerve root block is better than the group of Xubi prescription.There were no significant differences in pain VAS score,low back pain JOA score and concentration of IL-6 in serum between the two groups at 6 months postoperatively(P>0.05).The two groups had the same therapeutic effect.After Postoperative 12 month two groups of patients with pain VAS score,low back pain JOA score and concentration of IL-6 in serum had more significant difference(P<0.05),and the effect of the group of transforaminal selective nerve root block decreased,which the group of Xubi prescription is better than the effect of group of transforaminal selective nerve root block.The total fine rate of the group of Xubi prescription was 90.63%at 12 months postoperatively,the one of the group of transforaminal selective nerve root block was 66.67%.ConclusionTwo kinds of treatment selected by the research which can effectively improve the symptoms and signs of the lumbar disc herniation of Xubi type.From the chang of pain VAS score,low back pain JOA score and concentration of IL-6 in serum,the study found which the effect of group of transforaminal selective nerve root block is better than the group of Xubi prescription at the short-term effect of the first three months of the two groups;the two groups had the same therapeutic effect at 6 months postoperatively;the group of Xubi prescription is better than the effect of group of transforaminal selective nerve root block at the long-term curative effect of 12 months postoperatively.The two groups on the basis of combining traditional Chinese and western medicine theory effectively,which brought out the best in each other,is worthy of application in clinic practice.
