| ObjectiveTo observe the clinical therapeutic effect of Baihuangganning mixture combined with Transmetil on patients with Jaundice from liver cirrhosis induced by hepatitis B(damp-heat blood stasis syndrome),through randomized controlled study,including the observation of clinical indicators changes,the decrease of serological indicator,and the adverse reactions in the course of treatment.Methods1.Grouping methods: In this study,all the 90 cases of patients with Jaundice from liver cirrhosis induced by hepatitis B(damp-heat blood stasis syndrome)came from Digestive department outpatient and Liver Disease department outpatient in Wuhan Integrated TCM and Western Medicine Hospital,compounding into the standard set.According to the principle of random distribution,these cases were divided into three groups: combination of Chinese and western medicine group(Baihuangganning Mixture+Transmetil),western medicine group(Transmetil)and Chinese medicine group(Baihuangganning mixture).Each group has 30 cases.2.Treatment methods: The three groups were given basic treatment(hepatoprotective drugs such as glutathione,and routine antiviral therapy(Enticavir),infusion of plasma albumin or other supportive treatment when necessary).The combined treatment group were extra given Transmetil pills(500mg each time,two times per day)and Baihuangganning Mixture(composition: wormwood,gardenia,rhubarb,Radix Gentianae,radix paeoniae rubra,Rhizoma imperatae,Patrinia,Desmodium,bupleurum,Magnolia,hawthorn,medicated leaven,licorice.50 ml each time,two times per day).The western medicine group were treated with Transmetil pills(500mg each time,two times per day)on basic treatment.The traditional Chinese medicine group was treated with Baihuangganning Mixture(50ml each time,two times per day).And all the groups were treated for a period of 4 weeks.3.Main observation indexes: The traditional Chinese medicine(TCM)symptom changes,the decrease of Liver function indicators(TBIL,TBA,ALT,AST,?-GT and ALP,mainly for TBIL and ?-GT),security indicators changes,and the adverse reactions in the course of treatment.4.Statistical methods: SPSS 17.0 was used for statistical analysis.“P<0.05” means the difference has statistically significant,otherwise,“P > 0.05” means no statistically significant.Measurement data,expressed in((x|-)±s),used single factor analysis of variance in the comparison between groups and used paired t test in the comparison within group before and after treatment.Enumeration data,expressed in number of cases or percentage(%),used chi-square test(?~2).Ranked data Used H test and pairwise comparison used Nemenyi test.Results1.Comparison of total effective rate: The total effective rate of the combined group,western medicine group and Chinese medicine group were 89.3%,46.7%,48.3% respectively.The total effective rate had no statistically significant between western medicine group and Chinese medicine group after treatment(P>0.05).However,the total effective rate of combined treatment group was higher than both western medicine group and Chinese medicine group,and the difference was statistically significant(P<0.05).2.Comparison of liver function indexes: After 4 weeks of treatment,the liver function indexes(TBIL,ALT,AST,TBA,?-GT,ALP)of the three groups significantly decreased,and the difference had statistically significant(P < 0.05).The difference of the liver function indexes between western medicine group and Chinese medicine group was not statistically significant(P>0.05).The liver function indexes of the combined group were lower than those of the western medicine group and Chinese medicine group,and differences were statistically significant(P<0.05).3.Comparison of single TCM syndrome scores: The single TCM syndrome scores were significantly improved in three groups after treatment compared with before treatment,the differences were statistically significant(P<0.05).Comparing the results of the three groups after treatment,the single TCM syndrome scores of the combined group were lower than those of the western medicine group and Chinese medicine group,and differences were statistically significant(P<0.05).There were no statistically significant differences in symptom scores of the body and eyes stained yellow,yellow urination,nausea and vomiting,fatigue between the western medicine group and Chinese medicine group(P>0.05).The symptom scores of abdominal distension,hypochondriac pain,loss of appetite,dry mouth in the Chinese medicine group were lower than those of the western medicine group,and the differences were statistically significant(P<0.05).4.Comparison of the total score of TCM symptoms: There was no significant difference in the total symptom score of the three groups before treatment(P>0.05).Compared with the score before treatment,the total score in each group decreased after treatment and the difference is statistically significant(P<0.05).The syndrome score of the combined group was obviously lower than that of traditional Chinese medicine group and Western medicine group(P<0.05).The score of Chinese medicine group was lower than that of Western medicine group,and the difference had statistically significant(P<0.05).5.Security indicators evaluation: The security indicators,including vital signs,blood?urine?bowels routine tests,renal function tests,electrolytes tests,the color ultrasound examination of liver and gallbladder and so on,didn't change obviously.The mild diarrhea symptoms happened in two patients and got better after stopping taking Chinese medicine.Beside this,there is no depravation occurred in all the cases during the treatments.ConclusionBaihuangganning Mixture combined with Transmetil,used to treat patients with Jaundice from liver cirrhosis induced by hepatitis B(damp-heat blood stasis syndrome),can obviously fade jaundice,promote the recovery of Liver function and improve TCM symptoms.And its effect is much better than pure western medicine treatment and Chinese medicine.Besides,this treatment method is safe. |
