The Clinical Observation About The Efficacy Of Electroacupuncture In Thetreatment Of Chronic Pruritus And Itseffect On Thelevels Of IL-10 And TNF-? In Serum

Objective:Chronic pruritus is a common symptom of many skin diseases and systemic diseases.It has a high incidence and a longer course of disease,which seriously affects the patients' quality of life.Due to the complex etiology and pathogenesis of chronic itch,it has become a major problem in clinical treatment.In this study,we tried to use different frequencies of electroacupuncture(EA)treatment of chronic pruritus and observe the changes of the SCORAD score,DLQI score and ES score before and after treatment.Evaluated the clinical effect of high and low frequencies of EA on chronic pruritus.At the same time,the level of immune factors(TNF-?,IL-10)in serum was measured,and the changes of immune factors before and after treatment were compared.By this way,we aim to provide a new way for clinical treatment of chronic pruritus.Method:35 patients(includeing inpatient and outpatient)who were diagnosed chronic pruritus(including eczema,atopic dermatitis,neuro dermatitis,prurigo nodularis,unexplained chronic eczema like skin disease)were selected from the Tongji Hospital,Huazhong University of Science and Technology,Wuhan City Hospital of traditional Chinese and Western medicine,Wuhan Central Hospital,Wuhan Fifth hospital.They were randomly divided into high frequency EA group,low frequency EA group and drug group.EA were applied on ipsilateral Quchi,Hegu,Yinlingquan,Xuehai in two EA groups.After armaturing and “deqi”,Quchi and Hegu,Yinlingquan and Xuehai were separately connected with a pair of electrodes.High frequency EA group was treated with 100 Hz electroacupuncture.Low frequency EA group was treated with 2Hz electroacupuncture.The drug group was given cetirizine hydrochlo ride.One course lasted for 7 days,a total of 2 courses of treatment.We recorded all patients' symptoms,signs,SCORAD score,DLQI score,ES score before and after treatment,then established a database.The blood serum of all patients were collected centrifuged,and stored in the refrigerator at-80?before and after treatment.The corresponding indexes were detected by enzyme linked immunosorbent assay(ELISA).Spass11.5 statistical software were used to statistically analyze the SCORAD score,DLQI score,ES score,and concentrations of serum IL-10 and TNF-? in patients.Result:1.The total efficacy comparison after treatment: total effective rate of high frequency EA group was 100%,the low frequency EA group was 70%,the drug group was 60%;high frequency EA group got better effect than the other two groups and the differences were statistically significant(p<0.05).2.Comparison of SCORAD score,DLQI score and ES score before and after treatment: The SCORAD score and DLQI score were reduced after the treatment in the three groups and the differences were statistically significant(p<0.05).The ES score was reduced in high frequency EA group and drug group after treatment,the differences were statistically significant(p<0.05),but low frequency EA group did not have this effect.The effect of high frequency EA group on improving the three scores were more significant compared with low frequency EA group and drug group after treatment and the differences were statistically significant(p<0.05).3.The serum levels of IL-10 and TNF-? were compared before and after treatment: the levels of IL-10 in serum of the three groups were higher than those before treatment,and the level of TNF-? was decreased.But only the difference of EA group was statistically significant before and after treatment(p < 0.05).After treatment,the levels of IL-10 and TNF-? in high frequency EA group were significantly lower than those in other two groups(p<0.05).Conclusion:1.The effect of electroacupuncture treatment on chronic pruritus curative is remarkable,the highfrequency EA group curative is better than low frequency EA group,low frequency EA group curative is better than western medicine control group.2.The three groups of patients with chronic itch,skin lesions,pain and other emotions have a certain role in improving,but only the effect of high frequency EA group was more significant.3.The mechanism of electroacupuncture treatment on chronic pruritus may be achieved by increasing the level of inflammatory cells due to IL-10 and inhibiting the expression of inflammatory cytokines TNF-?.
