Retrospective Study Of Clinical Observation About Banxia Xiexin Decoction Treatment Of Chronic Gastritis

Objective:To investigate the current status to ascertain the inland developing trends by statistical analysis of clinical observation papers on Banxia Xiexin decoction treatment of chronic gastritis,in order to provide references for its further clinical development.Methods:All the literatures based on the clinical observation of the Banxia Xiexin decoction treatment of chronic gastritis published during 2007-2016 were searched in CNKI(China National Knowledge Infrastructure).Typed the relevant factors in the database using Excel 97-2003,and then analyzed the literature's chronological distribution,the clinical trial design(including sample size,random principle,control principle,blinding methods,interclass equilibration,method of statistical analysis,the follow-up and adverse reaction,recurrence rate,complication,side effect),diagnostic standard,inclusion/exclusion criteria,differentiation of disease,syndrome classification of tradition Chinese medicine,test medication(Decoction/Chinese patent medicine /Western medicine),standard of the curative effect evaluation,analysis of the clinical curative effect of the papers according to the qualification were sorted and analyzed by the methods of literature metrology,evidence-based medicine and statistical methods.Results: 1.There were 232 articles of clinical observation reference on Banxia Xiexin decoction treatment of chronic gastritis were collected,24312 cases intotal.2.The design of the clinical trial:there were 107 articles of the sample size were between 60 and 99 cases,about 46.2% of the total;190 articles applied the random principle,about 81.9% of the total;164 articles had the control group,about 70.6% of the total;121 articles did the comparison of the balance before the test,about 52.2% of the total;only an article applied double blinding methods;13 ones had mentioned complication;and only one explained the side effect;7 articles had the follow-up researches,about 3.0% of the total;58 articles had adverse reaction,bout 25.0% of the total.3.There were 145 articles without the diagnostic criteria,about 62.5% of the total;122 articles without the inclusion/exclusion criteria,about 52.6% of the total.There were 226 articles used the change of clinical symptoms as curative effect evaluation,about 97.4% of the total;196 articles used the change of endoscopic results and 43 ones based on the pathological findings as curative effect evaluation;39 articles used Hp clearance rate and 26 ones used the symptom score as curative effect evaluation.4.There were 126 articles specific the type of the chronic gastritis,about 54.3% of the total;which 63 ones were chronic atrophic gastritis and 54 ones were chronic superficial gastritis,about 27.2%/23.3% of the total;21 articles had two type of chronic gastritis;and 15 articles were related to the Hp infection chronic gastritis.5.There were 195 articles had clincal type,and 35 types in total.The top three were cold and heat syndrom,disharmony between liver and stomach and spleen stomach damp heat syndrome;while the virtual inclusion syndrom and demonstration and deficiency syndrome were 44.9%?34.6% and 20.5% respectively.6.There were 245 herbs which appeared 5490 times in all,the most frequently used was pinellia,liquorice,coptis and atractylodes(more than 200 times);codonopsis,scudellaria baicalensis,radix bupleuri,tangerine peel,white paeony root and chinese-date straighted after(more than 150 times).For the types of herbs,supplementing deficiency,removingheat,regulating qi was mostly used.The mostly joint used decotion is Zuojin Pill and Sini Powder.7.The mostly used western medicines were omeprazole,amoxicillin and clindamycin(more than 40 times);the second ones were bismuth potassium citrate,lansoprazole and rapeprazole;the frequently used Chinese patent medicines were Wei Fu Chun Pian and Sanjiu Weitai Granules.8.Analysis of curative effect:Banxia Xiexin decoction had significant clinical effect on chronic gastritis.Conclusion: 1.The number of the articles of clinical observation reference on Banxia Xiexin decoction treatment of chronic gastritis appeared rising trend,make big progress in research.The results showed that the clinical study design of the literature had some shortcomings.The simple size was relatively small;the concern about cilnical key factors including blinding methods,the follow-up and adverse reaction,recurrence rate,complication,side effect was a deficiency.Clinical diagnosis and curative effect evaluation standard was lack of evidence-based medicine,adopt less Guideline.The results of the research would be biased.2.Banxia Xiexin decoction could relieve the stomachache,ventosity and hiccup;also eliminate Hp;promote repaire the gastric gland;recovere the gland atrophy or intestinal metaplasia;and it showed significant clinical effect on chronic gastritis.
