The improvement of the basic medical insurance play an important role for maintaining residents' health and solving the difficulty of expensive medical treatment problem.However,residents' medical expenses is getting higher and higher in recent years,reasons are various,basic medical insurance is one of the reasons.Then how the basic medical insurance affects the residents' medical expenses? How to improve the basic medical insurance to alleviate the burden of medical expenses? It needs to be studied and solved seriously.The essays mainly adapts philological research and comparative study,through qualitative analysis and quantitative analysis research methods,also the theory and empirical analysis of the method,systematically analyzing how the basic medical insurance affects the residents' medical expenses in Shandong Province.Firstly,the conceptions,mechanism of action and theoretical basis are expounded;Secondly,residents' basic medical insurance and medical expenses is analyzed and found out the problems;By using two model method,the data of the survey questionnaire in Shandong has carried on the empirical analysis,the influence of the basic medical insurance on residents' medical expenses is found out.Finally,propose that,how to reduce the burden of residents' medical expenses and perfect the basic medical insurance.Through the research,the paper draws the main conclusions as follows:(1)Shandong Province positive responses national policy,firstly carries on experimental work of the basic medical insurance,at present has established the basic medical insurance system covering urban and rural areas,and has obtained rapid development.However,many issues still exist;for example,the fund balance rate of the basic medical insurance is higher,low security standards,the rapid increasing medical cost and other issues.(2)The health care expenses of urban residents always higher than that of rural residents,the percentage of the medical expenses in rural areas to consumption expenditure obviously higher than that in urban areas.There is a huge gap ofresidents' medical consumption level between rural and urban areas in Shandong Province.(3)In recent years,public hospital outpatient medical expenses and hospital ization medical expenses is higher than private hospitals in Shandong Province,and the proportion of outpatient medical expenses and hospitalization expenses for medicine is very high.(4)Residents' personal health spending accounts for higher percentage of the total health expenses.In 2014,residents' personal health spending accounts for 33.21 percentages of the total health expenses,far more than 15-20% of the world health organization standards,residents still face the problems of Difficulty and high cost to access medical service.(5)Different basic medical insurance affect residents' medical expenses differently.Compared with the uninsured population,basic medical insurance system for urban employees has indistinctive negative impacts on the hospitalizing rate,expenses of people who participate in basic medical insurance for urban employees under 10% level increased by 3.23 percent than the uninsured,this is because time cost of people who participate in basic medical insurance for urban employees is higher,in the case of a disease is not serious,people tend to a form of self-medication.When there are medical expenses,the residents' medical needs may increase or people tend to choose higher prices medical scheme,because urban employee basic medical insurance compensation level is high and the risk of moral hazard and other factors;Basic medical insurance system for urban residents has indistinctive positive impacts on the hospitalizing rate,to residents' medical expenses has no significant negative impacts,it indicates that the policy effect of basic medical insurance for urban residents may not be obvious.New rural Co-operative medical system increased hospitalizing rate to a certain extent,and under 5% level reduced rural residents 'medical expenses by 21.24 percent,it indicates that new rural Co-operative medical system really increased medical demand and reduced the residents' medical expenses burden to a certain extent.(6)The other factors influencing the residents' medical expenses: there isobviously positive correlation between severity of the disease and hospitalizing rate,and the severity of the disease has prominent impacts on medical expense.There is obviously negative correlation between hospitalizing rate and income.People aged 56 to 65 under 10% level has positive relationship with hospitalizing rate,increased by17.81 percent than people aged 16 to 35;In addition,medical expense of people aged46 to 65 and above 65 years old significantly increased than people aged 16 to35.Medical expense of married people significantly increased by 70.26 percent than divorced and widowed people.There is obviously negative correlation under 10%level between educational attainment and hospitalizing rate,hospitalizing rate of people with a high degree reduced by 11.48 percent than illiteracy and semiliterate.(7)In order to alleviate the financial burden of the medical,it must improve the basic medical insurance system.Basic measures that improve the basic medical insurance system: Increase government financial input,improve health standard,and establish mechanisms for the monitoring and evaluation,improve the efficiency of using Medical Foundation;Strengthen the management of pay of the basic medical insurance;Speed up the integration of basic medical insurance system,realize urban and rural overall development.Additional measures that improve the basic medical insurance system: deepen the reform of medical and health system,more attention paid to impartiality;Pay attention to disease control and prevention of the aged;Deepen the reform of medical system,abolish drugs addition policy;Establish a diversified medical market,make the residents' medical consumption more reasonable.
