ObjectiveThis study aimed to investigate the clinical application of perioperative multimodal preventive analgesia for alleviating postoperative pain in patients with liver cancer.MethodsThe 90 patients with liver cancer who undergoing partial hepatectomy were randomly divided into two groups as experiment group(multimodal preventive analgesia group)and control group using the random number table method,and each group had 45 cases.In experiment group,40 mg parecoxib sodium was injected intravenously 30 min before anesthetic induction,and 150 mg ropivacaine(0.375%)combined with 5 mg dexamethasone were applied to transversus abdominis plane block before closing abdomen.The patients in control group without above treatment.Patient controlled intravenous analgesia(PICA)was applied to all patients after surgery.Three days after surgery,40 mg parecoxib sodium was injected intravenously,twice a day for all patients.Postoperative pain scores were evaluated,and postoperative adverse events were observed for all patients.The number of cases of postoperative ambulation(>6 h for every day),time of flatus and defecation,and duration of hospital stay were recorded for all patients.ResultsThere were statistically significant differences in the interactions of the group,time,and group-time for the postoperative pain scores(Fgroup =39.01,P <0.001,Ftime =283.71,P<0.001,Fgroup-time =13.39,P <0.001).The postoperative pain scores in day 2 were significantly lower than that in day 1(F =21.72,P <0.001).No significant difference was found between in day 3 and day 1 in the postoperative pain scores(F =2.04,P >0.05).The number of cases of postoperative ambulation(>6 h)in experiment group was significantly more than that in control group(?2=11.250,P =0.001 for day 1;?2 =13.846,P(27)0.001 for day 2;?2 =5.378,P =0.020 for day 3).The time of flatus and defecation,and duration of hospital stay in experiment group were significantly shorter than that in control group(Z =3.992,P <0.001 for flatus;t =7.346,P <0.001 for defecation;Z =8.068,P <0.001 for hospital stay).ConclusionMultimodal preventive analgesia effectively alleviate the postoperative pain,benefits early ambulation,improves recovery of gastrointestinal function,and shortens duration of hospital stay in patients with partial hepatectomy for liver cancer.
