Object1. To explore the clinical effects of transitional care with TCM (Traditional Chinese Medicine) characteristics on daily living, neurological deficit, motor function, anxiety, depression, the quality of life,nursing satisfaction, mastery of knowledge of the disease, frequency of clinic visit and the compliance for stroke patients after discharge.2. The TCM nursing and transitional care were combined to explore the transitional care model with TCM characteristics which was suitable for stroke patients after discharge.Method1.During the period from October 2014 to December 2015,152 cases of stroke patients in department of Neurology of Teaching Hospital of Chengdu University of TCM were randomly divided into experimental group of 76 cases and control group of 76 cases with DPS software.2.Intervention method:Control group received conventional discharge instructions and weekly telephone follow-up. The experimental group carried out transitional care with TCM characteristics on the basis of the control group, namely the establishment of multi-disciplinary teams, developing health education with chinese characteristics at one week before discharge, performing compliance guidance at three days before discharge, application of family visits at first week,4 weeks,8 weeks,12 weeks after discharge. Assessing the patient's physical condition, guiding and implementing the nursing skills of TCM, the meals based on syndrome differentiation,the emotional nursing, rehabilitation training, daily life and medication nursing during home visit and clinic visit.3.Data collection:Collection of data at 1 week before discharge and three months after discharge. (1) General data:name, age, sex, marital status etc. (2) Therapeutic effect assessment index:Barthel Index (BI), China Stroke Scale (CSS), the Mos 36-Item Short Form Health survey (SF-36), Fugl-Meyer Motor Function Assessment (FMA), Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale(HAMD), admissiom and discharge satisfaction, knowledge of disease master score, clinic visits.4.Statistical process:Using independent t-test to describe quantitative data with Normal Distribution and/test to qualitative data by SPSS19.0 software.,test level is a= 0.05.Results1.After intervention with transitional care with TCM characteristics,the BI scores,FMA scores,CSS scores,SF-36 scores,satisfaction scores,frequency of clinic visit and knowledge scores about disease in experimental group wre more higher than control group,the difference was statistically significant (P<0.05).2.The CSS scores,HAMA and HAMD scores in experimental group are more lower than control group after intervention,the difference was statistically significant (P <0.05).Conclusion1.The transitional care with TCM characteristics can improve the emotional disorders (anxiety and depression),motor function,activities of daily living,quality of life, eurological deficitin, master of disease knowledge and satisfaction in stroke patients after discharge, which is more effective than the routine one.2. The frequency of clinic visit in experimental group wre more higher than control group,witch improve patient' compliance and the concern and attention to medical and health.
