| Since the implementation of the New Rural Cooperative Medical Scheme (NCMS) in 2005, the problem of "being difficult and costly to get medical treatment" has been alleviated to some extent. The social phenomena like "poverty caused by illness" and "return to poverty due to illness" have been reduced with the further implementation of NCMS. Thus, NCMS has made remarkable achievements. However, such issues have yet to be studied:whether the basic medical service level can meet the medical needs of the rural residents, whether the implementation of NCMS can improve the level of basic medical services in rural areas.Taking the public governance theory as the theoretical basis, the rural basic medical service’s scope and level and NCMS’s feature of benefiting the people as the object of study, an empirical analysis study was made based on specific situation of investigation in Minhou County. Existing problems in rural basic medical service under the NCMS system were analyzed. Then, countermeasures and suggestions were put forward, from the perspective of moderate responsibility of the government and the market.Through descriptive statistics, binary logistics regression analysis and qualitative research, the factors that impact the level of the basic medical services in Minhou County were explored. After the organization & analysis of questionnaire data and interview results, it is found that medical equipment, consulting platform, financial investment and compensation system have great influence on the construction of medical service environment. Moreover, all four factors are not able to perfectly meet the needs of doctors and patients. Diagnosis and treatment plan, nursing maturity and treatment effects pose great influence on the professional skills of medical personnel. Doctor-patient communication and disease tracking pose great influence on the attitude of medical service. Through factor analysis and interviews, it is concluded that there is still a large room for improvement on the level of medical services in Minhou County under the NCMS system.Finally, suggestions on improving the basic medical services in rural areas under the NCMS system were put forward as follows:In one hand, clarify the government’s responsibility in the medical service; coordinate the allocation of medical resources; construct tight management mode; strengthen the construction of grassroots personnel team; favor the general practitioner in performance wage distribution and so on; build tight management mode; sink human resources; strengthen the publicity of medical knowledge; improve patient’s health knowledge-ability; make full use of all kinds of carriers; build information sharing platforms; In the other hand, optimize the basic medical service system in rural areas through the market so as to offer a higher-levered medical service:bring in proper competition mechanism; give full play to supplemental protection role of commercial insuranc8e; encourage private funds to invest in it; expand the financing channels; raise medical service efficiency. At last, the basic medical service in rural areas could be both manageable and market-oriented through the cooperation between the government and the market. |
