Group B hemolytic streptococcus(GBS)is a pathogen that is associated with maternal and neonatal morbidity and mortality.It is the main cause of early onset severe infection and severe complications in neonates.There are a lot of studies on prenatal screening of pregnant women with 35-37 weeks and the corresponding preventive use of antibiotics,but the correlation between prenatal screening of GBS and pregnancy outcome is still rare at home and abroad.There were regional differences in GBS infection,and there were no similar reports in Shanghai area.Therefore,this study intends to examine the cervical secretion of Group B Streptococcus(GBS)during middle pregnancy and late pregnancy,and to observe the pregnancy outcome,and to provide evidence for the prevention and treatment of pregnancy.Objective: To investigate the relationship between GBS infection and pregnancy outcome in different gestational weeks.Methods:We choiced 104 cases of screening GBS positive pregnant womenfrom September 2015 to August 2016 in Shaghai Pudong hospital among of 744 pregnant women gynecology clinic regular antenatal examination,and then press GBS to detect gestational weeks were divided into 4 Subgroups: Group A 19 cases(detection in 20-24 gestational weeks);Group B 35 cases(detection in 24-28 gestational weeks);27 cases in Group C(detection in 28-34 gestational weeks);23cases in Group D(detection ingreater than 34 weeks of gestation).During the same period,46 cases pregnant women who cervical secretion GBS test negative in our hospital were selected as Control Group.Two groups of pregnant women were no other complications of pregnancy complications.Follow up each pregnancy pregnancy outcomes including gestational age groups,there is no pollution,premature rupture of membranes,postpartum hemorrhage,neonatal asphyxia,neonatal birth weight and maternal amniotic fluid infection,delivery and pathological examination after the existence of chorioamnionitis.Result:1.There was no significant difference between the Study Group and the Control Group.2.In this study,pregnant women with GBS carrier rate of 14% different gestational weeks were 12.4% detection of GBS carrier rate(Group A),15.6%(Group B),14.4%(Group C),13.1%(Group D),each gestational week GBS carrier rate was basically the same,no significant difference(p>0.05).3.The incidence of adverse pregnancy outcomes in the Study Group and the Control Group were 47.1% and 19.6% respectively.Among them,premature rupture of membranes were 41.4% and 17.4%;chorioamnionitis were 15.4% and 4.4%;prenatal infection were 28.9% and 6.5%;15.4% and 6.5% were premature,the Study Group was significantly higher than that of the Control Group,and there were significant differences(P<0.05).4.In the Study Group of GBS positive pregnant women,the incidence rate in each Sub Group of premature rupture of membranes,postpartum hemorrhage,and amniotic fluid pollution showed no significant difference(P>0.05).The incidence rate of patients in Group A and Group B of antepartum infection were(52.6% and 28.6%),neonatal infection respectively(15.8% and 17.1%),stillbirth or abortion(10.5%,8.6%).Comparison of the incidence of C Group and D Group of pregnant women infected(18.5% and 21.7%),neonatal infection(3.7% and 0%),stillbirth or abortion(3.7% and 0%),were significantly increased;A Group and B Group in these types of adverse pregnancy outcomes was higher than that in Group C and Group D;of pregnant women in Group A;B,C,the incidence rate in chorionic membrane(10.5%,17.1%,14.8%),premature birth rate(15.8%,17.1%,18.5%)was higher than that in Group D(the incidence rate is respectively 8.7%,8.7%)(P<0.05).Conclusion: GBS colonization during pregnancy can lead to adverse pregnancy outcome,long time infection may increase the rate of mother and child infection,pay attention to prenatal screening,early detection,timely treatment of GBS infection,may improve the outcome of pregnancy.
