ObjectivesThis study was conducted to observe clinical outcomes of characteristic therapy of Traditional Chinese Medicine on enhanced recovery after total knee arthroplasty.It aimed to provide basic evidence of effectiveness and safetyfor better clinical decision for characteristic therapy of Traditional Chinese Medicine on enhanced recovery after surgery.MethodsA randomized clinical trial was conducted on 60 patients under enhanced recovery after total knee arthroplasty from February 2016 to February 2017 in the Third Affliated Hospital of Guangzhou University of Traditional Chinese Medicine.Cases were devided into treatment group and Control group.Clinical data including age,gender,education,BMI index,PQSI sleep score,time of farting,time of getting out of bed,KSS score and VAS score were recorded and statistical analyses were conducted on both groups.Results1.Knee society function score:Scores of both group were increasing.On the pre-op day:Treatment group:53.03±4.80,Control group:52.90±4.54;No differences were statistically significant(P>0.05).On the 3rd post-op day:Treatment group:68.93 ± 3.22,Control group:67.47±1.50;Differences are statistically significant(P<0.05).On the 7th post-op day:Treatment group:81.40±2.79,Control group:79.73 ± 3.47;Differences are statistically significant(P<0.05).On the 14th post-op day:Treatment group:87.97±2.09,Control group:87.10 ± 2.17;No differences were statistically significant(P>0.05).2.Pain VAS score:Scores of both group were decreasing.On the pre-op day:Treatment group:7.00±0.95,Control group:7.07±0.91;No differences were statistically significant(P>0.05).On the 3rd post-op day:Treatment group:4.13 ± 0.90,Control group:3.87 ± 0.90;No differences were statistically significant(P>0.05).On the 7th post-op day:Treatment group:3.43±0.50,Control group:3.37 ±0.49;No differences were statistically significant(P>0.05).On the 14th post-op day:Treatment group:3.03±0.49,Control group:2.90 ± 0.55;No differences were statistically significant(P>0.05).3.Pittsburgh sleep score improvement:On the pre-op day:Treatment group:9.93 ± 2.45,Control group:10.30 ± 2.55;No differences were statistically significant(P>0.05).On the 14th post-op day:Treatment group improvement:3.70 ± 2.02,Control group improvement:2.37 ± 2.87;Differences are statistically significant(P<0.05).4.Complications:5 patients suffered from swollen lower limb and 2 patients suffered from stiffness of lower limb in the treatment group,rate of complications being 23.33%;8 patients suffered from swollen lower limb and 3 patients suffered from stiffness of lower limb in the treatment group,rate of complications being 36.67%.No infection or delayed wound closure or DVT,etc happened in both groups.5.Time of farting:Treatment group:9.70 ± 3.72h,Control group:18.4 ±4.12h;Differences are statistically significant(P<0.01)6.Time of getting out of bed:Treatment group:2.23±0.82d,Control group:2.93 ± 1.17d;Differences are statistically significant(P<0.01)7.Medical cost:Treatment group:70854.68±8068.11RMB,Control group:76286.52±6483.30RMB;Differences are statistically significant(P<0.01)ConclusionBoth groups had a satisfactory recovery.The application of characteristic therapy of Traditional Chinese Medicine on enhanced recovery after total knee arthroplasty was effective by accelerating recovery gastrointestinal function and improving post-op sleep quality.It can also help improve knee function after total knee arthroplasty.
