Objective: To observe the clinical effect of the Sangbaipi Decoction and Xiaochengqi Decoction treated Acute Exacerbation Of Chronic Obstructive Pulmonary Disease(AECOPD)patients with syndrome of phlegm-heat obstructing lung and intestinal qi stagnation. The result may offer guides in curing AECOPD with the management of clearing away lung-heat,resolving phlegm and cleansing intestinal tract. Methods: Seventy hospitalized AECOPD patients(Stage ?-?)by differentiation of phlegm-heat obstructing lung and intestinal qi stagnation were randomly and equally divided into the treatment group and the control group. The patients of treatment group taken Sangbaipi Decoction and Xiaochengqi Decoction which have the efficacy of clearing away lung-heat,resolving phlegm and cleansing intestinal tract, and simultaneously accepted conventional therapy of the western medicine same as the control group(controllability oxygen treatment,bronchodilators,expectorants,anti-infection treatment). The course of treatment was 14 days.To compare the result of two groups by analyzing TCM primary symptoms, syndrome integral,CAT integral, lung function, arterial blood gas analysis and effective rate, and the differences between the two groups were statistical analyzed. The safety index:blood routine examination, routine urine test, stool routine examination(include occult blood),liver function and renal function,electrolytesdetection.Results:(1)TCM primary symptoms: After the treatment, the primary symptoms scores in the treatment group, such as cough, the volume of sputum, the difficulty of expectoration, the property of sputum, gasp, abdominal distension and constipation, were lower than that in the control group. The treatment group was better than the control group in improving cough, the volume of sputum, the difficulty of expectoration, the property of sputum, gasp, abdominal distension and constipation(P<0.05).(2)Syndrome integral:After the treatment, the syndrome integral in the treatment group was lower than that in the control group. The treatment group was better than the control group in improving syndrome integral(P<0.01).(3)The treatment group had the total effective rate of 94.12%.The control group had the total effective rate of 81.82%.The clinical efficacy in the treatment group, compared with that in the control group, had a statistically significant difference(P<0.05).(4)CAT integral: After the treatment, the CAT integral in the treatment group was lower than that in the control group. The treatment group was better than the control group in improving CAT integral(P<0.01).(5)Lung function:After the treatment, the FEV1% and FEV1/FVC in the treatment group, compared with that in the control group, had no statistically significant difference(P>0.05).(6)Arterial blood gas analysis: After the treatment, the PaO2 and PaCO2 in the treatment group, compared with that in the control group, had a statistically significant difference(P < 0.05).The treatment group was better than the control group in improving PaO2 and PaCO2.(7)This study had no obvious side effects on the human body,so the clinical safety was well.Conclusions:(1)Sangbaipi Decoction and Xiaochengqi Decoction could obviously improve the primary symptoms, such as cough, the volume of sputum, the difficulty of expectoration, the property of sputum, gasp, abdominal distension and constipation,in the AECOPD patients with syndrome of phlegm-heat obstructing lung and intestinal qi stagnation. It could obviously reduce the syndrome integral and increase the total effective rate of treatment.(2)Sangbaipi Decoction and Xiaochengqi Decoction could effectively reduce the CAT integral in the AECOPD patients with syndrome of phlegm-heat obstructing lung and intestinal qi stagnation. So the quality of life of patients was improved.(3)Sangbaipi Decoction and Xiaochengqi Decoction could obviously improve the PaO2 and PaCO2.So it could reduce carbon dioxide retention and increase blood oxygen concentration.
