Clinical Features And Morphological Study Of Mirror Aneurysms

ObjectiveThe purpose of this study is analyzing the clinical characteristics and morphological parameters of the mirror aneurysms,providing reference for predicating the rupture risk of the mirror aneurysm and the treatment strategy for mirror aneurysm.MethodsCollecting the clinical data of the patients who were diagnosed as mirror aneurysms in our hospital from January 2015 to December 2016.Screening the patient according to the inclusion and exclusion criteria.For the enrolled patients,analyze their clinical features and measure the morphological parameters of aneurysms through 3D-DSA.Compare the morphological difference between the ruptured and unruptured aneurysms.Results22 patients were included in the study.All of them were female.The mean height of ruptured group was 4.82mm ± 2.18mm,while the mean height of unruptured group was 3.14mm ± 1.92mm(P=0.010.The mean aspect ratio of ruptured group was 1.57±0.61,while the mean ratio of unruptured group was 1.10±0.45(P=0.007).The mean size ratio of ruptured group was 1.64 ± 0.56,while the mean size ratio of unruptured group was 1.04 ± 0.70(P=0.003).Aneurysms with daughter cysts in ruptured group accounted for 73%.Aneurysms with daughter cysts in unruptured group accounted for 9.3%(P<0.01).There were no significant differences between the ruptured and unruptured groups in the morphological parameters such as length,maximum internal diameter,size of aneurysm,height/length,neck width and diameter of parent arteries.Patients were followed up for 3-27 months,2 cases of ruptured aneurysms which were treated by Interventional embolization recrudesced.2 cases of unruptured aneurysms were enlarged.ConclusionsMirror aneurysms show obvious female predominance.And they usually occur in posterior communicating artery,communicating segment of internal carotid artery and middle cerebral artery.The aneurysms which have greater height,greater aspect ratio,larger size ratio are more likely to rupture.Both interventional embolization and clipping can have good curative effect,the clinical treatment should be based on individual circumstances of the patients.
