Objective:Mirror-like intracranial aneurysms(MirAn)is a rare type in clinic.There are few related studies while the clinical characteristics,treatment options and prognostic factors are still much controversy.The aim of this study was to analyze the related data in our center in order to discuss the clinical features,individualized treatment and prognosis of mirror-image aneurysms.Metholds:We retrospectively analyzed 61 cases of MirAn diagnosed between May 2014 and January 2019 in the neurosurgery department.There were 12 males and 49 females,aged from 45 to 77,with an average age of 60.62±7.80 years old.To analyze the clinical features of MirAn.,and univariate analysis and multivariate logistic regression analysis was used to analyze the effects of different treatment methods and timing on the prognosis of patients with MirAn.Results:1.45(73.78%)patients had a higher Glasgow Outcome Scale(GOS)score(4~5 points),indicating good prognosis,and 16(26.22%)patients had a lower GOS score(1~3 points),indicating poor prognosis.2.Univariate analysis showed that preoperative CT Fisher grade and Hunt-Hess grade of the MirAn patients were associated with prognosis(P < 0.05).The CT Fisher grade and Hunt-Hess grade were higher,the prognosis was even worse.3.Multivariate logistic regression analysis showed that Hunt-Hess grade of MirAn patients was associated with prognosis(P < 0.05),while other factors such as treatment measures were not associated with prognosis(P > 0.05).Conclusion:The factors affecting the prognosis of patients with mirror-like intracranial aneurysms are complex.The most important factors are preoperative CT Fisher grade and Hunt-Hess grade.For patients with MirAn,the responsible aneurysms should be accurately judged,and appropriate individualized treatment schemes should be developed according to the patients’ Hunt-Hess grade,preoperative CT Fisher classification,aneurysm factors(shape,size,location)and their own conditions,so as to improve the surgical efficacy and obtain good prognosis.
