Analysis Of Intermediate Care Demands Of Patients With Acute Stage After Discharge And Evaluation Of Existing Care Systems

Purpose:To study the demand for intermediate care of patients who discharging from large hospitals and to evaluate the effect of community-based intermediate care model"family ward" in Hangzhou.Methods:A total of 107 hospitalized patients in the Department of Neurology,Surgery,Elderly and Rehabilitation of the three large hospitals in Hangzhou were selected as the object,and the questionnaires of demands were collected and counted.A total of 19 patients were selected from two community hospitals,evaluating before and after intermediate care with SF-36 scale for quality of life score,also the care process and content were recorded.Results:All patients had at least one of the above requirements at discharge,with an average requirement of 4.78.There was a significant negative correlation between the Barthel score and the demand(r =-0.30,p<0.05),and the acceptance attitude was significantly correlated with the demand(p<0.05).Drug guidance,sequelae awareness,lifestyle guidance,rehabilitation guidance,recurrence risk identification;Rehabilitation of damaged parts,drug guidance,related surgical operation,diet nutrition guidance,referral guidance;Rehabilitation guidance,drug guidance,basic disease management,first aid knowledge,social needs ranks top 5 in Neurology,Surgery,Rehabilitation and Geriatric department respectively."Family Wards" intermediate care model can significantly improve the overall quality of life of patients,physiological function,general health status,energy,social function(p<0.05).Physiological function,physical pain,emotional function,mental health were not significantly changed(p>0.05).Conclusion:The assessment of daily living ability score and acceptance attitude are important factors influencing demand.Different departments,diseases determine different types of demands,but the status of drug demand is high,elderly patients should also be concerned about their social needs.The existing "family ward,intermediate care modelin Hangzhou can improve the quality of life of patients...
