Development Of Family Relocation Stress Scale During Patient Transferred From Intensive Care Units

Objective: To develop a set of scale for measuring family relocation stress during patient transferred from ICU.The scale is to provide an objective tool for ICU staves assessing and dealing with the family member's relocation stress.Methods:(1)A draft scale was developed based on the related scale abroad,literature reviewing and semi-structured interviews for 12 relatives of ICU patient.(2)A total of 7 nursing experts were selected by Delphi method for two turns of consultation.We calculated the questionnaire response rates,the authority coefficient and the harmony coefficient.The mean and standard deviation,the coefficient of variation were used to item selection.The pretest scale formed.(3)145 relatives of ICU patient from 4 hospitals in Chongqing were investigated by pretest scale.The discrete tendency,factor analysis,correlation coefficient and Cronbach'? coefficient were used to item selection.The final scale formed.(4)186 relatives of ICU patient from 4 hospitals in Chongqing were investigated by final scale.The scale's internal consistency was tested by using Cronbach'? coefficient.Exploratory factor analysis and confirmatory factor analysis were used to test the validity.Results:(1)The initial construction of the scale entries includes separation anxiety,environmental changes,disease severity,nursing model changes,the safety of transfer,self-efficacy of care ability of the 6 dimensions(first grade indexes),and 30 items(second grade indexes).(3)7 nursing experts were enrolled in the assessment system investigation by Delphi.The expert's positive coefficients of 2 rounds were 100% respectively,authority coefficient was 0.83.The harmony coefficients of first grade indicators were 0.36,0.36,second grade indicators were 0.35,0.37,P<0.05.Combined with selection of indicators and expert opinion,the pretest scale includes 6 first grade indicators and 25 second grade indexes.(4)Five items were deleted by the statistical analysis with pilot investigation.6 first grade indicators(transfer anxiety,environmental changes,disease severity,nursing model changes,the safety of transfer,self-efficacy of care ability)and 20 second grade indicators.(5)The Cronbach'? coefficient was 0.882.Exploratory factor analysis extracted 6 items,accord with the scale's dimensions,accumulated variance contribution rate was 72.226%.Confirmatory factor analysis showed ?2/df=1.677,comparative fit index=0.936,Tucker-Lewis index=0.921,root mean square error of approximation=0.061.Conclusion: This scale analyzed by Delphi and item analysis could comprehensively reflect characteristic of the family's relocation stress.The scale has good reliability and validity.It can be used as the evaluation tool for family relocation stress during patient transferred from ICU.
