Objective:To investigate the effect and safety of drill-hole drainage and craniotomy with hematoma removal and decompression in the treatment of supratentorial cerebral hemorrhage(STCH)associated with aspirin so as to choose proper surgical method for this kind of patients.Methods:The clinical materials of sixty-two patients with STCH in our hospital from Jan.,2008 to Sept.,2014 were analyzed retrospectively.The drill-hole drainage was performed in 34 cases(G1)and the craniotomy with hematoma removal and decompression in 28(G2).The GCS scores,hematoma size before&one week after operation,postoperative complications,mortality and GOS scores 6 months after operation were analyzed statistically.Results:?The average GCS scores one week after operation of G1 and G2 were both significantly higher(P<0.01)than the preoperative scores and the volumes of hematoma were both significantly decreased(P<0.01).?)The rates of re-bleeding in the two groups were 17.6%(G1)and 25%(G2)respectively,which had no significant difference.The re-bleeding both happened within 3 days after operation.? The rate of postoperative infection in G1 was significantly lower than that of G2(P<0.05).?The postoperative mortality(14.7%)in Gl was no significantly different from that(21.4%)in G2(P>0.05).?GOS score of G1 was significantly higher than that of G2 six months after operation.Conclusion:There is no significant difference on the short-term effects between G1 and G2.However,The former has markedly lower postoperative infection rate and better long-term effects than the latter.
