Objective:Benign prostatic hyperplasia(benign prostatic,hyperplasia,BPH)is a common disease of the urinary system in the middle and elderly age male patients.Diabetes mellitus(DM)is a group of metabolic disorder with long-term high blood glucose as the main feature and due to insulin deficiency and/or impaired insulin function leading to elevated blood glucose level.In recent years,with the improvement of people's living standard,the ratio of old population is more then before,and the incidence of hypertension,DM,obesity and BPH were also increased with the improvement of living standards.This project is designed to study the relationship between BPH and DM.Existing research showed that Metabolic disorder of the blood glucose has a certain relationship with the BPH in elderly male patients,but the number of sample is small.By reviewing the clinical analysis of BPH patients in our hospital physical examination center,this study aims to analyze the relation between the indexes and BPH,providing the basic knowledge for clinical diagnosis and intervention.We used a retrospective analysis of the clinical data of the ShanDong provincial hospital physical examination center of 1500 male patients age greater than 50,testing the pertinent indexes,examining the correlation of prostate volume and each index,analyzing influential factors associated with progression of effect of benign prostatic hyperplasia.Using a self-made questionnaire,we detailed record of the age of all study subjects and other general information.CX7 automatic biochemical analyzer is used to detect fasting blood glucose.Calculation of body mass index(BMI),subjects of barefoot,wearing only underwear measuring the height(m))weight(kg),BMI = weight(kg)/height(m2).The volume of prostate(V)was measured by abdominal ultrasonography.The formula is calculated as follows:V(cm3)=0.52*(Left and right diameter)*(Front and rear diameter)*(Upper and lower diameter)(cm).We statistic indexes of the selected 730 cases of BPH patients with blood glucose and other metabolic indices(FBG,ZhPBG,HbAlc,FPG,fins,HOMA-IR)and BPH evaluation index:such as prostate volume,serum prostate specific antigen(prostate specific antigen,PSA),International Prostate Symptom Score(International Prostate Symptom Score,IPSS),the course of benign prostatic hyperplasia(BPH).In accordance with the criteria for the diagnosis of BPH and DM,730 cases BPH patients were divided into simple BPH group(n = 330)and undergoing with BPH complicated with DM group(n = 400).Next we divided into groups according to the level of FBG,2hPBG,HbAIc,FINS,HOMA-IR of patients to analyzine the influence of blood glucose and other indicators to BPH progression.Statistical treatment of data for statistical analysis using SPSS 20.0,measurement data is showed by(x±s),use student's t test;The count data indealed with chi-square test and variables using multiple regression analysis,P<0.05 has statistical significance.Result1.Prostate volume,age,body mass index,systolic blood pressure and blood glucose level have significant positive correlation(P<0.05);The height,weight,blood pressure and blood lipids have no significant correlation(P>0.05);The level of BMI,FBG,HbA1c of benign prostatic hyperplasia patient is significantly higher than that in non benign prostatic hyperplasia patients(P<0.05).2.Simple BPH group PV is 39.24±16.65 ml,BPH combined with DM group PV is 53.35±14.26ml,BPH combined with DM group PV is significantly higher than that of BPH group PV.There is a significant difference(t=5.381,P=0.005);BPH group PSA is 1.88±0.98 ng/ml,BPH combined with DM group PSA is 3.46±1.12 ng/ml;BPH combined with DM group PSA is significantly higher than that of BPH group PSA;there is a significant difference(t=4.830,P=0.013);BPH group IPSS is 8.87±2.67;BPH combined with DM group IPSS is 13.12±4.25;BPH combined with DM group IPSS was significantly higher than that of BPH group IPSS.There is a significant difference.(t=3.792,P=0.010);BPH group duration is 12.79±3.672 years,BPH combined with DM group duration is 13.62±3.896 years,the duration of BPH combined with DM group is longer than that of the pure BPH group,but there is no significant difference(t=0.854,P=0.437).3.Fasting blood glucose levels were determined in accordance with 7mmol/L.Normal fasting glucose group BPH PV is 40.79±13.87 ml,impaired fasting glucose group BPH PV is 55.27±17.69ml,impaired fasting glucose group BPH PV is significantly larger than that of normal fasting blood glucose in BPH group PV,there is significant differences(t=4.465,P=0.002);Normal fasting glucose group BPH PSA is 2.04±1.65 ng/ml,impaired fasting blood glucose abnormalities in group BPH PSA is 3.35±2.87 ng/ml,impaired fasting glucose group BPH PSA is significantly higher than that of normal fasting blood glucose in group BPH PSA,there is significant differences(t=3.674,P=0.006);Normal fasting glucose group BPH IPSS is 8.78±3.76,impaired fasting glucose group BPH IPSS is 12.46±5.27,impaired fasting glucose group BPH IPSS is significantly higher than that of normal fasting blood glucose in BPH group IPSS,there is statistical difference(t=5.643,P=0.010);Normal fasting glucose group BPH duration is 12.65±3.58 years,impaired fasting glucose group BPH duration is 13.68±3.77 years,impaired fasting glucose BPH duration was higher than that of fasting blood glucose in normal BPH group,but no statistical difference(t=0.867,P=0.368).4.HbAlc determination is based on the 2009 ADA guidelines on the glycosylated hemoglobin(HbAlc)levels of(<7%),are divided into the normal group and the abnormal group.HbAlc abnormal BPH group PV is 53.79±17.34 ml,HbAlc normal BPH group PV is 41.57±11.41ml,there is statistical difference(t=4.465,P =0.002);There is no significant statistical difference between the level of PSA,IPSS,duration of HbAlc abnormal BPH group and HbAlc normal BPH group(t=0.843 P=0.517).5.The evaluation of the high level of insulin(HINS)is based on the FINS>15mU/L.Hins abnormal BPH group PV is 56.57±26.75 ml and Hins normal BPH group PV is 44.79±17.54 ml,statistically significant difference(t=3.774,P =0.017);Hins abnormal BPH group BPH disease is 18.12±5.76 years,Hins normal BPH group BPH disease is 13.88±4.65 years,statistical difference(t-3.825,P=0.031).Two groups of patients with PSA and IPSS score no significant statistical difference(P>0.05).6.According to the HOMA-IR>2.8 as the standard of insulin resistance.HOMA insulin resistance index is calculated as[Plasma glucose(GLU,mmol/L)*Serum insulin(mIU/L)]/22.5.And insulin resistance of BPH patients PV is 54.68±16.77ml,insulin sensitivity in BPH group PV is 41.35±12.67 ml,the two groups have statistical difference(t=4.327,P = 0.004);Insulin resistance in BPH patients with benign prostatic hyperplasia(BPH)course is 16.79±4.43 years,insulin sensitive BPH patients were BPH disease is 12.78±3.76 years,the two groups have statistical difference(t=4.367,P=0.036).There is no significant difference between the level of PSA and IPSS of the wo groups of patients(P>0.05).Conclusion:1.Age,body mass index,systolic blood pressure,blood glucose levels are closely related to the changes of prostate volume.Moreover,BMI,FBG,HBA1C are probably the risk factors for the development of the disease in patients with benign prostatic hyperplasia.2.It can possibly retard the development of the disease in patients with BPH with routine screening for blood glucose and taking positive measures to the BPH patients complicated with diabetes mellitus.
