Acute Pulpitis refers to the dental pulp tissue of Acute inflammation,the main characteristic of clinical manifestation is paroxysmal,spontaneous pain.Acute pulpitis can also produce severe toothache without any stimulus,and can be alleviated by itself.Treatment of acute pulpitis of traditional way is drainage,but drainage can not eliminate the inflammation,it only alleviates the high pressure of the medullary cavity,the common method to control the pain clinically is pulpectomy or pulp devitalization under local anesthesia.The viewpoint of modern endodontics is that the best way to control the pulpitis pain is the thorough pulpectomy and cleaning the pulp tissue in root canals completely.Due to the age-related changes of the elderly,the pulp cavity and root canal are coarctation,even with varying degrees of calcification and atresia,especially in the back teeth,and most of the elderly patients suffer from systemic disease,have poorer tolerance of root canal treatment and difficult to cooperate for a long time treatment,The purpose of this study was to investigate the treatment methods in pain control to elderly patients with acute pulpitis,through evaluate the efficacy and the acceptance degree of chair-time.Objective To evaluate the analgesic effect of two different methods on pain control through treating acute pulpitis of molars with aged patients with pulpectomy or pulp devitalizeation under local anesthesia.Methods To choose 204 first molars and second molars with acute pulpitis or exacerbation of chronic pulpitis in aged patients who were at the age of 75~89,and divided them into two groups randomly,All patients were under local anesthesia and each molar of the experimental group was drained and sealed devitalized material(Antipulp),while that of the control group underwent pulpectomy and sealed calcium hydroxide paste,all the patients were required to return visit in 7days.The patients were supposed to evaluate their pain intensity in the first diagnosis and referral by using the VAS pain scale and mark the pain intensity on the line.to compare the time required for the first treatment,efficacy,remission rate of pain,the pain degree,the incidence of EIAE after first visit.To remove the temporary sealing at the second visit,the experimental group had the pulp extirpated,the control group had the intracanal calcium hydroxide removed.Then explored the root canal of the two groups,to compare the degree of pain in canal at root canal exploration in subsequent visit.Results The two groups were effectively relieve and there were there were varying degrees of endodontic interappointment pain after the first visit.There were no significant differences in the two groups in efficacy,remission rate of pain,the pain degree,the incidence of EIAE after first visit(P>0.05),there were significant differences in the time required for the first treatment(P<0.05),and there were significant differences in the degree of pain in canal at subsequent visit in two groups(P<0.05).Conclusion Two methods achieved analgesic effect for aged patients with acute pulpitis on molars,but could not avoid EIAE.Antipulp inactivation required less time in the first visit and got less pain in the second visit compared with pulpectomy,so it could be recommended for the treatment.
