Subjects: 1.Investigating the tissular organization and pathological changes in ablation area of human nodular goiter tissues in vitro after treated by radio frequency ablation(RFA). 2. Discussing the influence on regular pathological diagnosis of thyroid tissue after operated by RFA. 3. Using the transmission electron microscope to observe the ultrastructure of human thyroid tissue in vitro after RFA.Methods: 1.RFA was performed in 8 pieces of the resected human thyroid tissues in vitro with single-needle and monopolar automatic power output. The pathologic and ultrastructural changes of ablated thyroid area were observed. 2. Taking retrospective analysis on pathological statistics of 135 specimens from Jan 2014 to Jan 2016 operated by thyroid RFA treatments, selecting 20 specimens with preoperative and postoperative pathological biopsy to compare whether there is consistency between them.Results: The shape of ablated zone profile on human thyroid tissue in vitro after RAF seems like an ellipse with dark yellow or gray white color and distinguishable boundary between ablated and unablated tissue. From peripheral to central area of profile is external ablated area, ablated area and ablated-needle area. Under HE-stained light microscopy shows that along the needle trail there is an obvious cellular necrotic area without follicular epithelial cell nucleus and quantities of lymphocytes infiltrating. Under transmission electron microscope shows that the junction between adjacent follicular epithelial cells can not be recognized, along with pyknosis, chromatin disintegration, heterochromatin clusters, cavernous degeneration of rough endoplasmic reticulum and unidentified cell organ(Mitochondria, Golgi apparatus, etc.). There is no diagnostic difference between preoperative and postoperative thyroid tissues. The diagnostic accuracy rate is 100%.Discussion: 1.Observed with light microscopy the basic pathological structure and characterized cellular form of thyroid tissue are well conserved after automatic RFA, and the pathologic diagnosis of ablated and unablated area are consistent. 2.The ultrastructure of thyroid follicular cells became degeneration and necrosis after RFA, suggesting the effectiveness of RFA in thyroid nodule treatment. 3.No difference was found between preoperative and postoperative pathological diagnosis after RFA treatment on benign goiter. 4. Tissue samples obtained from coarse needle biopsy after radiofrequency ablation of thyroid nodules can be obtained from the real diagnosis.
