and upper dermis,which may cause various skin damage and its manifestation mainly are red spots and papules.Patients with eczema suffer from severe itching and allergies.Allergens sensitization,impaired skin barrier function and abnormal immune function caused by the imbalance of Th subsets differentiation are major pathogenetic factors of eczema.Interleukin-32(IL-32),as a kind of preinflammation cytokine and OX40 L as the ligand of the T-cell costimulatory molecule,are both play important roles in Th subsets differentiation and functioning of various cytokines and chemokines.In this study we will preliminarily discuss: 1.The differences of expression levels of IL-32 m RNA and OX40 L m RNA in peripheral blood between patients with eczema and normals;2.The differences of expression levels of IL-32 m RNA and OX40 L m RNA between the patients of different genders;3.The expression differences of IL-32 m RNA and OX40 L m RNA in peripheral blood among different types of eczema patients;4.The correlation between the expression of IL-32 m RNA and OX40 L m RNA,and the interaction of the two factors on the pathogenesis of eczema;5.The correlation between the expression levels of IL-32 m RNA and OX40 L m RNA in peripheral blood of eczema patients and Eczema Area and Severity Index(EASI)and Dermatology Life Quality Index(DLQI);6.The expression differences of IL-32 m RNA and OX40 L m RNA in peripheral blood of eczema patients after two-week and four-week Total Glucosides of Peony(TGP)treatment to analyze therapeutic mechanism of TGP on eczema;7.The correlation between the expression levels of IL-32 m RNA and OX40 L m RNA in peripheral blood of eczema patients and some laboratory indexes.Methods:In the study,we chose 60 cases of eczema from the Tianjin Union Medicine Centre outpatient service as the experimental group.According to the history and manifestations,the 60 cases were dividid into acute,subacute and chronic eczema group,20 cases in each.The diagnostic criteria of eczema is on the basis of themonograph of Clinical Dermatology edited by Zhaobian.40 cases were chosen as normal controls from the Gerneral Hospital of Tianjin Medical University.We used Real-time fluorescence quantitative reverse transcription polymerase chain reaction(RT-PCR)method in this study to detect the expression levels of IL-32 m RNA and OX40 L m RNA in peripheral blood of 60 cases with eczema and 40 normal persons respectively.Marks were given to 60 cases according to the EASI scoring criteria and DLQI scoring criteria.The statistical software SPSS17.0 was used to process experimental data,with P<0.05 was statistically significant.Results:1.The expression levels of IL-32 m RNA and OX40 L m RNA in peripheral blood of eczema patients was higher than those of the normal controls(4.08±0.98 vs 9.95±0.33,P<0.05;2.46±0.92 vs 9.74±0.37,P<0.05).2.There was no significant difference of the expression levels of IL-32 m RNA and OX40 L m RNA in peripheral blood of between different genders(both P >0.05).3.The expression level of IL-32 m RNA of acute,subacute and chronic eczema group decreased in turn(2.91±0.42,4.35±0.63,4.98±0.19,P<0.05);The expression level of OX40 L m RNA of acute,subacute and chronic eczema group decreased in turn(1.39±0.29,2.71±0.49,3.28±0.58,P<0.05).4.After 2-week treatment by TGP capsules,the expression levels of IL-32 m RNA and OX40 L m RNA in peripheral blood of patients were lower than their first visits and the EASI score and DLQI score decreased,and all differences were statistically significant(IL-32⊿CT:5.95±0.84 vs 4.24±0.98;OX40L⊿CT:5.20±0.65 vs 2.58±0.99;EASI score:8.66±3.01 vs 13.02±2.32;DLQI score:12.06±3.63 vs 16.84±3.80).After4-week treatment,the expression levels of IL-32 m RNA and OX40 L m RNA in peripheral blood of the patients were much lower than 2 weeks ago with decrease of the EASI score and DLQI score,and all differences were statistically significant(IL-32⊿CT:8.44±0.69 vs 6.11±0.72;OX40L⊿CT:8.21±0.76 vs 5.28±0.61;EASI score:5.73±2.15 vs 8.79±3.20;DLQI score:7.68±3.12 vs 12.01±3.70).5.The expression levels between IL-32 m RNA and OX40 L m RNA in peripheral blood of the patients were positively correlated and differences were statistically significant(r=0.80,P<0.05).The expression levels of IL-32 m RNA and OX40Lm RNA in peripheral blood of the patients were both positively correlated with EASI score(r=-0.74,r=-0.70,P<0.05).6.The expression levels of IL-32 m RNA and OX40 L m RNA in peripheral blood of the patients were positively correlated with the eosinophils(EOS)(r=-0.03,r=-0.02),and all differences were statistically significant(P<0.05).The expression levels of IL-32 m RNA and OX40 L m RNA have no obvious relationship with white blood cells and lymphocyte count(P>0.05).Conclusions:1.The expression levels of IL-32 m RNA and OX40 L m RNA in peripheral blood of eczema patients were higher than those of healthy controls and had positively correlations with EASI score and DLQI score,which demonstrated that IL-32 and OX40 L might participate in the pathogenesis development of eczema and might have an promoting effect on eczema to some degree.2.The expression levels between IL-32 m RNA and OX40 L m RNA were positively correlated,which showed the two factors might promote each other through TSLP pathways.3.After 2-week and 4-week treatment by TGP capsules,The EASI score and DLQI score decreased and the expression levels of IL-32 m RNA and OX40 L m RNA both decreased in the eczema
