A Survey Of Essential Public Health Service In Some Rural Areas In Tianjin

ObjectiveThis study was used to prepare the implementation of the Essential Public Health Service survey of residents(hereinafter referred to as the resident questionnaire) and Essential Public Health Service implementation questionnaire of the medical workers(hereinafter referred to as: the medical worker questionnaire). The part of Tianjin rural residents and medical workers in rural essential public health services field investigation for the project implementation. According to the attitude from area residents and medical workers and the basis of detailed data, we should improve implementation and enhance the project to make the statistics and comprehensive analysis, that included some corresponding improvement countermeasure and the suggestion of essential public health services in Tianjin.Subjects and methodEssential public health services survey in the three counties was conducted in Tianjin, from July 2015 to January 2016. There were 697 households and 75 medical workers. Each subject was asked to fill out an anonymous questionnaire by themselves.The resident questionnaire and medical worker questionnaire were compiled by us according to the national essential public health service specification(2011).Investigators that had been unified trained filled in the investigated residents and medical workers questionnaires independently.And then we sorted and recycled the Questionnaires uniformly.Questionnaires were entered using Epidata Entry 3.1 and were rechecked to ensure the accuracy of the data. The statistical analysis was conducted using the SPSS 18.0 statistical package.ResultResidents filled in the questionnaires from the rural areas in JiXian, Baodi Area and Jinghai District.They were permanent residents that lived for at least 5 years or more. The resident questionnaire was issued to 697 households. The recycling questionnaires were 694, 99.6% of residents all issued questionnaires, included 677 effective questionnaires, 97.6% of all residents recycled questionnaires. The recycling questionnaire were 75, 100% of medical workers all issued questionnaires, included 75 ffective questionnaires. The compliance survey was good.Resident valid questionnaires from JiXian, Baodi Area and Jinghai District and the number of questionnaires respectively was 187, 196 and 187. The 677 Respondents were from 25 to 55, the average(39.56±1.62) years of age, included 310 men(45.8%) and 367 women(54.2%).Medical workers valid questionnaires from Ji Xian, Baodi Area and Jinghai District, and the number of questionnaires respectively was 22, 24, and 29. A total of 75 medical workers, the average(34.18±1.49) years of age, of which 36 men(48%) and 39 women(52%). 45.3% of medical workers were from towns and townships, 54.7% of medical workers were from village clinics.Two types of people surveyed for essential public health services witting rate respectively reached 90.1% and 96.0%; Residents satisfaction for basic public health services was only 59.4%, and the medical workers for basic public health service satisfaction rate was 96.0%. Residents from questionnaire, Tianjin rural areas were investigated for some traditional service project implementation in line with Tianjin urban basic or difference was not big, but for some chronic diseases such as severe mental illness and health management of colorectal cancer. There were still some not ideal project implementation, such as constitution of traditional Chinese medicine identification(12.3%) of all households surveyed, life style and health assessment(8.2%).Can be seen from the medical personnel questionnaire in the essential public health services, there were still difficulties of project implementation.The elderly health management implementation difficulties was 78.7% of all medical workers.The health management implementation difficulties of traditional Chinese medicine was 72.7%.The management implementation difficult for patients with severe mental illness was 72%. At the same time, in addition to the present stage had been set up basic public health services outside, we still needed to add other items.The medical workers selected add all residents once a year of free physical examination project(93.3%), selected add 82.7% of colorectal cancer screening, etc. And the medical workers investing funds for basic public health services were also not satisfied, only 16% medical workers surveyed think 40 yuan per capita or basic can completely meet the current basic public health services cover the service content, up to 74.7% of the workers surveyed thought salary was low.ConclusionNational essential public health services in Tianjin rural areas basically surveyed in full swing, especially on the traditional service items, but some have been carried out in Tianjin urban district and the Binhai New District of basic public health services were not in the surveyed area, needed to establish the basic public health project once a year, which had all residents free of charge, such as colorectal cancer screening. At the same time, there were still some questions on the implementation of the services, such as severe mental illness, and colorectal cancer project were missed in the actual situation, the elderly health management, management of patients with severe mental illness such as not smooth project implementation process. Notable was, the basic public health services in terms of money still couldn't meet the demand of medical workers, with the constant improvement of the economic level, therefore, should continue to increase investment in capital, increase the use of health resources. Tianjin rural areas still needed to further advance the basic public health services to carry out the situation, paid attention to the traditional project at the same time, conquer not mature services.According to the surveyed area residents and medical workers needed to add new projects, and increased the funds required for the project.
