Objective: To evaluate precise hepatectomy in clinical curative effect of recent for the treatment of primary liver cancer.Methods: Through the computer system to retrieve the Pub Med, Medline, the Cochrane library databases, CNKI and Wanfang databases to identify randomized controlled trialsnd controlled trials that assessed the efficacy of precise hepatectomy and Conventional hepatectomy in treatment of primary liver cancer. Using Rev Man5.3 software for two kinds of operative methods in intraoperative and postoperative curative effects and prognosis of indicators for Meta analysis.Results: Participants included 6 RCT and 9 CCT, a total of 1367 cases of patients. Compared to conventional liver resection, precise liver resection significantly reduce the intraoperative blood loss(MD=-174.13, 95%CI:-233.20~-115.06, P<0.00001).Reduce the amount of perioperative blood transfusion(MD=-311.04, 95%CI:-444.53~-177.55, P<0.00001).Shorten the hospital stay(MD=-3.11, 95%CI:-4.36~-1.87, P<0.00001), reduces the residual ratio of margin(OR=0.18, 95%CI:0.17~0.45, P=0.003), speed up the recovery of liver function: ALT(MD=-311.04, 95%CI:-444.53~-177.55, P<0.00001), AST(MD=-67.27, 95%CI:-94.36~-40.18, P<0.00001), TBIL(MD=-4.54, 95%CI:-7.33~-1.75, P=0.001), ALB(MD=3.75, 95%CI:0.54~6.97, P=0.02), reduce the incidence of complications(OR=0.24, 95%CI:0.17~0.35, P<0.00001). Reduce the 1-year recurrence rate(OR=0.56, 95%CI:0.37~0.85, P=0.007). Improved one-year survival rate(OR=1.61, 95%CI:1.01~2.57, P=0.007). But no statistically significant difference in perioperative blood transfusion rate(P=0.12),and extend the operation time(MD=31.72, 95%CI:12.00~51.44, P=0.002).Conclusion: Precise liver resection treatment for primary liver cancer were compared to Conventional liver resection can completely remove the lesion, reduce the 1 year recurrence rate, increase 1 year survival rate, accelerate the recovery of liver function, shorten hospitalization time, reduce the postoperative complications, thus has better clinical effects in the near future.
