Objective: In order to understand the basic demographic information and sickness status of type 2 diabetes mellitus inpatients in different citizens in Tianjin from 2010 to 2014. Analyze the number and type of complications and the construction of hospitalization expenses. Besides, to sutdy the variation trend and influence factor of hospitalization costs. And provide theoretical support for reducing the hospitalization costs and medical insurance policys finally promote the management of diabetes mellitus inpatients in China.Methods:We conducted a 50 percent randomly sample from Tianjin medical insurance database system from January 1, 2010 to December 31, 2014. At last 742711 high quality type 2 diabetes mellitus cases were collected. The basic demographic and hospitalizaion information were described at first. The hospitalizaiton costs and the variation tendency of construction were analysied in five years. Rank sum test were carried to explore the the single factor of hospitalization cost in patients with different complications. Multiple Linear Regression Method and Structural Equation Model were adopted to research the factors of hospitalization and the relationships of these factors. All the analyses were completed with SAS 9.2 and AMOS 17.0.Results: Most diabetes mellitus patients suffered from more than one complications in Tianjin from 2010-2014. And the most common complication was Coronary heart disease(CHD). The prevalence of complications in common residents were lowre than employees. The medicine expenditure, examination fee and treatment costs were the major apartment of hospitalizaiton costs in diabetes mellitus patients. The hospitalizaiton costs of residents which grew sustainedly were lower than the employees. The more complications, the higher hospitalization costs. The hospitalization costs were elevated more apparently by macroangiopathy. Hospital level was directly related to the hospitalization costs. The diabetes mellitus patients who chose the thtertiary hospitals, who were elder, employees and male had higher hospitalization costs. What's more, the factors of hospitalization costs in residents and employees were different, so were the reliationships of these factors.Conclusion: The complications were common in type 2 diabetes mellitus patients in Tianjin and most patients chose tertiary hospitals. Compared with employees diabetes mellitus patients, residents patients suffered from few complications and lower hospitalization costs. Medicine expenditure, examination fee and treatment costs constituted vast majority of hospitalization costs which was unreasonable. The number and type of complications, hospital level, age and sex were influence factors of hospitalization costs. In order to improve the utilization of health resources and reduce the economic burden of diabetes patients, corresponding health insurance policys should be made for residents and employees respectively.
