Objective: To explore whether computer navigation technology have the advantages of improving effect of postoperative rehabilitation,we compared the therapy one year after operation between navigation groups and traditional groups. Methods: We divided 60 patients into two groups randomly from September 2013 to September 2014.one group of them were performed with the computer-Assisted technology and the others were chosen with traditional technology. We had made the statistical analysis about these KSS scores,WOMAC scores,Oxford scores and LTFA,ROM,,the displacement of knee joint in Coronal plane, the displacement of knee joint in sagittal plane, hospital stays after operation, blood loss in perioperative period, operative time, blood clots and infection which were collected from each patient.Results:60 cases were followed up for an average of 12 months(12-14 months). The LTFA in navigation group was 179.97°±1.07°and the LTFA in traditional group was 181.00°±1.99°,showing significant difference(t=-1.711,P<0.05)Patients in navigation group just needed(6.83±1.44) days to reach the target that ROM of knee joint should be from 0°-100°(hospital stays after operation),but the patients in traditional group needed(9.17±1.34)days to achieve the goal, and the times between two groups had significant difference. The ROM of knee joint in navigation group was 104.83°±6.20°,and the other group was 101.47°±5.83°, showing significant difference. The difference between the medial gap and lateral gap of knee joint in flexion which was less than 5mm in navigation group existed 27,and the traditional group had 22.these statistical differences between two groups had statistical significance. The difference between the medial gap and lateral gap of knee joint in extension which was less than 5mm in navigation group existed 28,and the traditional group had 22.These statistical differences between two groups also had statistical significance. If the displacement of knee joint in Coronal plane and in sagittal plane were both less than 5mm,the navigation group appear 25 cases and the traditional group appear 24 cases at same time, showing no significant difference.29 patients in navigation group walked up and down without crutch one year after surgery, and traditional group had 28 patients. The operative time in navigation group was(103.90 ? 19.12) minutes, comparing with traditional group about(84.23±11.43) minutes. The amount of bleeding in perioperative period of navigation group which was(1002.80±230.36)ml showed no significant difference with amount of bleeding in traditional group.Three kind of scores(WOMAC score?KSS score?Oxford score) between two groups had obvious differences..Four patients in traditional group suffered from blood clots and the other group had found four cases, which had no significant difference. There was no pulmonary embolism occurred. The navigation group had occurred 2 superficial infections, and traditional group had occurred 1 superficial infections.None of them had occurred articular cavity infection. Conclusion: The computer-assisted total knee replacement has larges of advantages in improving accuracy of the implant placement? accuracy of limb alignment?balance of soft tissue?ROM of knee joint in early stage?function of clinical scores. But there is still a need to study clinical curative effect of long-term about computer-assisted TKA.
