Research On Clinical Prognosis Evaluation Of Infiltrating Immune Cell Subtypes In Breast Infiltrating Ductal Carcinoma Patients

Abstract/Summary:
Objective: The immune status of tumor microenvironment is associated with the prognosis of patients with breast invasive ductal carcinoma,but the correlation among tumor infiltrating immune cell subsets and patient’s prognosis and curative effect is still not clear.In this study,by using immunohistochemistry staining intraoperative paraffin specimens.We analysis the correlation among tumor infiltrating immune cell subsets and the prognosis in patients with breast invasive ductal carcinoma.Methods: By using immunohistochemistry staining(CD8, CD20, CD68, FoxP3) antibodies on intraoperative paraffin specimens in 102 patients with breast invasive ductal carcinoma.The immunohistochemistry staining results being analyzed the distribution of tumor infiltrating immune cell subsets and their positive expression of infitrating immune cells within the center of tumor’stroma by two pathologists under the microscope.We counted the number of positive infiltrating immune cells in 3-5 highest density visions.Finally we analysis the correlation bwteen the data we obtained and the patient’s clinical data and survival prognosis.Results:1.By using the microscope,we observed that the distribution of the infil trating immune cells in the tumor centerare diffused and scattered.2.Univariate analysis showed high tumor-infiltrating CD8+ CTLs/CD20+ B lymphocytes infil tration and low tumor-infiltrating Fox P3+ Treg/CD68+ macrophages were related to good OS and DFS(P<0.05),traditional prognostic factors: ER positive status were related to good OS(P<0.05),SBR grade were related to good OS and DF S(P<0.05).3.Multivariate analysis showed high tumor-infiltrating CD8+ CTLs/CD20+ B lymphocytesinfiltration and ER positive status were related to good OS a nd DFS(P<0.05).Low tumor-infiltrating Fox P3+Tregs were related to good OS(P<0.05).High tumor-infiltrating CD8+CTLs were were related to good DFS(P<0.05). Lowtumor-infiltrating Fox P3+Tregs/CD68+macrophages and PR positive statuswer e related to good DFS(P<0.05)4.The survival analysis showed high tumor-infiltr ating CD8+ CTLs/CD20+ B lymphocytes infiltration and low tumor-infiltrating Fo x P3+ Treg/CD68+ macrophages increase the overall survival and desease-free s urvival of breast invasive ductal carcinoma(P<0.05);High tumor-infiltrating CD8+ CTLs/CD20+ B lymphocytes infiltration increase the overall survival and des ease-free survival of ER negtive breast invasive ductal carcinoma(P<0.05);High tumor-infiltrating CD8+ CTLs infiltration and lowtumor-infiltrating Fox P3+ Treg/CD68+ macrophages increase the overall survival and desease-free survival of lymphonode negtive breast invasive ductal carcinoma(P<0.05).Conclusion:Tumor-infiltrating immune cells both have the activities of antitumo r and protecting tumor, which dependent on the subtype of tumor-infiltrating i mmune cells.1.Tumor-infiltrating CD8+ CTLs/CD20+ B lymphocytes are associa ted with good prognosis,whereas tumor-infiltrating Fox P3+ Tregs/CD68+macroph ages are associated with adverse outcomes.Tumor-infiltrating CD8+CTLs/CD20+B lymphocytes might become independent predictive factors of breast cancer pati ents prognosis.In ERnegtive breast cancer patients,Tumor-infiltrating CD8+CTLs/CD20+ B lymphocytes are associated with good prognosis.In lymphonode negti ve breast cancer patients,Tumor-infiltrating CD8+CTLs are associated with good prognosis,while tumor-infiltrating Fox P3+Tregs/CD68+macrophages are associate d with adverse outcomes.
