Objective: To analyse the clinical features of primary hyperparathyroidism(PHPT);To explore the association between PHPT and metabolic syndrome(MS), body mass index(BMI), hypertension, hyperglycemia, dyslipidemia.Methods: We enrolled 61 patients with PHPT who were admitted in Fujian Provincial Hospital from January 2009 to November 2015. Retrospective analyses were conducted on the general data, clinical manifestation, biochemical indicators, and pathological examination of the subjects. There were 61 healthy subjects matching age and sex as controls. Patients were stratified in symptomatic and asymptomatic PHPT. To compare the difference of their occurrence of MS and its components,the association PTH and total calcium with MS and its components was analysed.Results:(1) Among 61 patients, 21 were males and 40 were females, mean age(50.36±15.32) years. There were 21subjects(34.4%) who were asymptomatic and 40 subjects(65.6%) were symptomatic, 19 subjects(31.1%) with bone symptoms. Among 53 surgical subjects, 43 subjects(81.1%) were adenoma.(2)There were differences of SBP, DBP, TG, HDL-C, UA, WBC and the prevalence of high TG, low HDL-C and MS between PHPT group and control group(P<0.05). There were differences of SBP, DBP, PTH, UA and the prevalence of hypertension and MS between asymptomatic PHPT group and symptomatic PHPT group(P<0.05). No differences in MS and its components were found between males and females(P>0.05).There were positive association between PTH and total calcium with TG(spearman correlation coefficient R 0.312, P=0.014; R 0.289, P=0.024, respectively), and inverse association between PTH and HDL-C, spearman correlation coefficient R was-0.292, P=0.023.Conclusion:(1) PHPT occurs mainly in females. The symptoms are diversified, with bone symptoms as the major clinical manifestation. The pathological type is primarily adenoma.(2) PHPT patients show increased incidence of MS. Moreover, the incidence of MS in asymptomatic PHPT patients was higher than that in symptomatic PHPT patients. Therefore, PHPT patient should be assessed for MS and its components.(3) There were no differences in MS between males PHPT patients and females PHPT patients.(4) There were positive association between PTH and total calcium with TG, and inverse association between PTH and HDL-C.
