Diagnosis And Surgical Treatmentof Primary Hyperparathyroidism

Objective: Discussion of different calcium levels in patients with primary hyperparathyoidism surgery and postoperative serum parathyroid hormone(PTH),calcium(Ca2+) variability.Methods: Randomly selected patients in our hospital in the past 10 years,30 cases of primary hyperparathyroidism,they were treated with surgery,3 patients were proved to parathyroid hyperplasia after surgery,the others were provedto Parathyroid adenoma.Divided into 3 groups according to serum calcium levels,10 patients in each group.They were taken before surgery(the first l day after admission),postoperative(after 30 minutes excision) blood,We used the ECL assay whole paragraph PTH value,According to serum parathyroid hormone,among the same group of patients with preoperative and postoperative comparison,The groups were compared between groups While monitoring blood Ca,P,and blood levels of alkaline hosphatase.Results: The first group of patients before surgery(the first day after admission l) and after(30 minutes after excision) blood PTH value compared to P1 <0.05(P = 0.000),there was a significant difference;P2<0.05(P= 0.032),there was a significant difference;P3<0.05(P= 0.000),there was a significant Difference,Signifcantly lower than the preoperative serum calcium,33.3% of patients with lower than normal calcium,Given calcium and active vitamin D3 therapy,A week later normal serum calcium.2)Patients with symptoms were relieved,Such as bone pain,joint pain,polydipsia,drinks,urinary and gastrointestinal symptoms improved significantly within1 week.3)Comparison between groups: The first group compared with the second group about the curative effect of operation,P1,2>0.05(P= 0.881),There was no significant difference;The first group compared with the third group P1,3>0.279(P= 0.881),There was no significant difference;The second group compared with the third group about the curative effect of operation,P2,3>0.189(P= 0.881),There was no significant difference.Conclusion: 1)Surgical removal of the patients with primary hyperparathyroidism is the only cure disease has the exact effect of measures.2) for the patients with primary hyperparathyroidism,Serum calcium and serum parathyroidhormone level is main indicators of the parathyroid function,Correlation between thechange.3)Serum parathyroid hormone levels can be the most sensitive and reliable reflects the parathyroid function,Postoperative serum parathyroid hormone levels decline to assess the degree of success of the procedure criteria.4)In patients with primary hyperparathyroidism before surgery no matter how calcium levels,surgical treatment is effective and no significant difference in efficacy.
