Objectives To study the influencing factors for controlled blood pressure in the very elderly hospitalized patients with hypertension.Methods Included 111 very elderly hospitalized patients with hypertension in the Affiliated Hospital of North China University of Science and Technology of Department of geriatrics in the April to September 2015. According to the monitoring blood pressure level, the blood pressure level below 150/90 mm Hg is divided into the controlled group, and blood pressure levels more than 150/90 mm Hg is divided into the un-controlled group. Biochemical indexes, mini nutritional assessment, Pittsburgh sleep quality score, and Hamilton anxiety score of the two groups were compared.Results 1 Body mass index, course of hypertension, classification of hypertension, proportion of smoking, proportion of alcohol, combined with diabetes proportion and culture degree had no significant difference between the un-controlled group and the controlled group(P> 0.05).Compared with the controlled group, the age of un-controlled group increased, the proportion of male, the proportion of regular medication, the proportion of physical exercise decreased, the difference is statistically significant(P<0.05).2 The un-controlled group and the controlled group in albumin, pre-albumin, total cholesterol, triglyceride, high density lipoprotein cholesterol, low density lipoprotein cholesterol and fasting blood glucose had no significant difference(P>0.05); two groups in blood uric acid level with statistical difference(P<0.05).3 There was no statistical difference in mini nutritional assessment short-form between the un-controlled group and the controlled group(P<0.05).4 Between the un-controlled group and the controlled group, the Pittsburgh sleep quality index, self evaluated sleep quality score, sleep time score difference were statistically significant(P< 0.01), and the sleep time, sleep efficiency, sleep disorders, hypnotic drugs, daytime function score were no significant difference(P>0.05).5 There were statistically significant differences in the scores of mental anxiety between un-controlled group and controlled group(P<0.05), while there was no statistical difference in the total score of Hamilton anxiety and somatic anxiety(P>0.05).6 Logistic regression analysis of the situation of controlling in the very elderly hospitalized patients with hypertension as the dependent variable, age, gender, smoking history, drinking history and other factors as the independent variable. Results showed hyperuricemia(OR=1.015, 95%CI:1.004~1.027), poor self rating sleep quality(OR=5.543, 95%CI:1.823~16.849) are risk factors for controlling blood pressure in the very elderly patients with hypertension; positive physical exercise(OR=0.162, 95%CI:0.042~0.625), regular medication(OR=0.070, 95%CI:0.020~0.248) and high MNA-SF score(OR=0.645, 95%CI:0.423~0.984) are protection factors for controlling blood pressure in very elderly patients with hypertension.Conclusions High blood uric acid level, sleep quality, physical exercise, regular medication and nutritional status are the influencing factors for controlled blood pressure in the very elderly hospitalized patients with hypertension.
