Objective To explore the major environmental risk factors for coronary heart disease(CHD), and preliminarily discuss the relationship between microRNA and CHD in expressions of microRNA in CHD cases and control cases, so as to provide the scientific basis for CHD prevention and risk assessment and management of cardiovascular disease; and to provide theoretical basis for further exploration of the epigenetic mechanism of coronary heart disease(CHD). Methods Hospital-based case-control study method was adopted. 280 cases and 280 controls were collected from People's Hospital affiliated to Fujian TCM University and First Hospital affiliated to Fujian Medical University from November 2014 to December 2015 strictly according to the inclusion criteria and exclusion criteria. The patients were given in-situ epidemiology investigation and molecular epidemiological investigation, namely questionnaire investigation, physical examination and molecular biology laboratory inspection. Questionnaire content included: general demographic characteristics(gender,age, marital status, educational background and occupation, etc.), individual behavior factors(smoking history, drinking history, history of drinking tea, history of diet and physical exercise, etc.), family history of cardiovascular disease, self-rating anxiety scale(SAS) and self-rating depression scale(SDS), etc. Physical examination included:body height, body weight, waist circumference, blood routine and blood biochemistry,etc. Laboratory test mainly extracts total RNA in peripheral blood leukocytes, and RT-FQ-PRT was used to determine non-coding RNA(miR-126, miR-143 and miR-145)expression in the case group and the control group. Logistic regression analysis(backward method) was used to explore whether the environment factors and small non-coding RNAs were independent risk factors for coronary heart disease(CHD).Results Multi-factor logistic regression analysis results showed that: overweight,anxiety, and elevated systolic blood pressure(SBP) were risk factors for CHD, and higher HDL, physical activity at work and thinner were the protection factors;Expression of miR-126, miR-143 and miR-145 in peripheral leukocytes of cases were all higher in CHD group than the control group, with statistically significant differences(P<0.05);(3) Multi-factor logistic regression results indicated that: after controllingother environmental risk factors, higher expression of miR-126 was independent risk factors for CHD. Conclusion(1) Coronary heart disease is the result of environmental and genetic combined action, and bad behavior lifestyle, psychological factors and genetic factors are the major risk factors for CHD, so all-round multi-level risk management should be carried out in accordance with the social- environmentpsychology- biological medical mode, so as to effectively prevent or control the occurrence or development of coronary heart disease(CHD).(2) After controlling other environmental risk factors, high expressions of miRNA-126 in peripheral leukocytes were independent risk factors for CHD, suggesting the non-codling small RNA might be associated with CHD, which still needs further research to explore.
