ObjectiveCoronary heart disease is heart disease caused by coronary atherosclerosis which causes stenosis or obstruction of the vascular cavity,or myocardial ischemia,hypoxia or necrosis due to functional changes in the coronary arteries.According to the 2017 Cardiovascular Disease Report,about 290 million people suffer cardiovascular disease,cardiovascular deaths make more than 40% of the deaths of residents that ranks first,higher than cancer and other diseases.Coronary heart disease has become the main cause of death in China.How to detect,diagnose and treat coronary heart disease earlier has become the focus of current research.miRNAs have been widely concerned in recent years.Some studies have found that miRNA-155 is closely related to the occurrence and development of coronary heart disease,and is expected to become a new target for the diagnosis and treatment of coronary heart disease in the future.This study will investigate the expression of microRNA-155 and the correlation with Gensini score during coronary atherosclerosis,and reveal the correlation between miRNA-155 and coronary heart disease and its value of clinical application.MethodThis study included eighty-seven inpatients which were enrolled in the Cardiovascular Department of the Second Hospital of Jilin University from October 7,2018 to November 30,2018.Coronary angiography showed that patients with any degree of stenosis?50% were enrolled in the coronary heart disease group,the total is 42(25 males and 17 females);patients diagnosed with type 2 diabetes according to clinical diagnostic criteria were enrolled in the diabetes group,the total is 29(17 males and 12 females),and patients who underwent catheter angiography of which showed no coronary artery stenosis were enrolled in the control group.it had 13 inpatients(5 males and 7 females).The degree of coronary stenosis was assessed by Gensini score.Gensini score ? 20 was considered as 1 group,Gensini score > 20 was considered as 2 groups.Therefore,the experimental group patients were divided into coronary heart disease group 1 and group 2,as well as diabetes group 1 and diabetes group 2.In the experiment,total RNA was extracted from the patient's plasma,and reverse-transcribed into cDNA,and the expression level of miR-155 in the plasma was detected by qPCR.Statistical analysis was performed by using SPSS 22.0.Result 1.Compared with the control group,the plasma levels of miR-155(0.34(0.12,1.61)vs 1.00(0.85,1.49),p<0.05)were down-regulated in patients with coronary heart disease;plasma in patients with coronary heart disease 2 miR-155(0.07(0.02,0.45)vs.vs 1.00(0.85,1.49),p<0.01),the expression of the second group was decreased in the two groups(p<0.05);2.In the diabetic group,the expression levels of miR-155(0.24(0.07,0.72),0.25(0.10,0.53)vs.1.00(0.85,1.49),p<0.05)were down-regulated in the plasma.The differences were statistically significant.3.Correlation analysis showed that there was a moderate negative correlation between plasma miR-155 and Gensini during coronary atherosclerosis(r=-0.527,p<0.01).4.ROC analysis of the predictive value of miR-155 in the formation of coronary stenosis showed AUC(95% CI;0.563,0.875.P<0.01),the value of optimal threshold is 0.681,sensitivity is 0.718,specificity is 0.8.5.Patients in the experimental group were divided into hypertension group,diabetes group,hypertension and diabetes group,none of hypertension and diabetes group.There was no statistical significance between each of two groups(P> 0.05).Conclusions 1.The expression level of miR-155 in plasma of patients with coronary heart disease was decreased,and it had a moderate negative correlation with Gensini score,and have a certain predictive value for coronary stenosis.2.The level of miRNA-155 expression in plasma is down-regulated in patients with type 2 diabetes,it may play a protective role of the heart.
