Clinical Value Of Variation Of The NT-proBNP In Severe HFMD Children

Objective Collected the plasma NT-pro BNP of the acute phase of the severe hand, foot and mouth disease. We are aim to discuss the prognostic value of NT-pro BNP in the children with severe hand, foot and mouth disease.Method 747 consecutive PICU and the infection ward admissions during the study period of March 2014 to July 2014 at Hunan Children's Hospital were enrolled in the study.The 747 patients were divided into myocardical injury group and non-myocardical injury group,survival group and death group.According to clinical registration from designed by ourselves,NT-pro BNP and LDH,CK,CKMB,c Tn I,WBC,Glu were measured in 24 h after admission.Compare NT-pro BNP level of groups,summarize the change of NT-pro BNP in groups,analysis the prognostic value of NT-pro BNP and NT-pro BNP combine WBC,Glu.Results(1) The levels of NT-pro BNP increases with severe hand,foot and mouth disease illness severity, the difference of boths groups were statistically significant(P<0.01).(2) The level of NT-pro BNP?LDH?CK?MB in myocardial injury group was higher than non-myocardical injury group, the difference of two group was statistically significant(P<0.01).(3)The level of LDH,CK,CKMB, Mb, CTn I increased with the NT-pro BNP, the difference had statistical significance;Log NT-pro BNP is positively related to log(LDH ? CK ? CK-MB ? MB ? c Tn I),and the correlation coefficient with logc Tn I is the biggest.(4) The levels of NT-pro BNP in the death group was significantly higher than the survival group, and the difference of two group was statistically significant(P<0.01).(5) HFMD Laboratory score increases with the illness severity, the difference was statistically significant(P<0.01).(6) HFMD Laboratory score value is divided into two groups by 5points, the difference of two groups prognosis value(cardiopulmonary failure,death,death within three days) was statistically significant(P<0.01),the survival rate is different.Conclusion1 NT-pro BNP could be used to help to assess severe hand, foot and mouth diseases condition and to predict prognosis.2 NT-pro BNP combine with WBC,Glu could be better reflect the severe hand, foot and mouth disease severity and prognosis.
