Clinical Observation On Treating Osteoarthritis By BUSHENHUOXUEJIEDU Method

Objective: We make use of the method of Bushenhuoxuejiedu to treat osteoarthritis according to the etiopathogenisis of osteoarthritis and observe the efficacy and safety of Gubi decoction. To study the mechanism of treating osteoarthritis preliminarily about Gubi decoction.Methods: 100 patients with osteoarthritis meeting the inclusion criteria were included and randomlu divided into two groups: 50 in the treatment group and 50 in control group. There is no significant difficence in gentle, age and course of patients in two groups. The severity extent and the clinical index were scored before treantment. The treatment group was given Gubi decoction + glucosaminesulfate + Diclofenac Sodium Sustained Release Tablets and control group was given glucosaminesulfate and Diclofenac Sodium Sustained Release Tablets for 4 weeks. The severity extent, the clinical index and laboratory index were compared to evaluate the efficacy and safety of Gubi decoction treating osteoarthritis. The serum and synovial fluid were collected and MMP-3, TIMP-1, IL-1?, TNF-? were detected to study the mechanism of Gubi decoction treating osteoarthritis preliminarily.Results:1 ?In the aspects of improving severity extent: The clinical efficacy rate of Gubi decoction treating osteoarthritis was 86% while the clinical efficacy rate of control group was 74.0%.2?TCM symptoms: The main symptoms disappear cases(%) of traditional Chinese medicine treatment are statistically significant differences(P<0.05). And the tongue and pulse condition and the number of cases of patients in two groups after treatment were lower than before treatment, there were significant difference of tongue and pulse condition after treatment with two group.3? In the aspects of improving the clinical index: The score of clinical index dominant syndrome of patients in treatment group were markedly alleviate and pain, Number of tenderness,swell, timeand active degree,Morning stiffness, ALSFRS and walk were improved.4? In the aspects of improving laboratory index: ESR and CRP were decreased significantly. 5? The results of mechamical study: Contents of MMP-3 and TIMP1 in serum and contents of IL-1? and TNF-? were decreased significantly in the treatment group. Gubi decoction can effectively improve TCM symptoms of OA patients, such as inconvenient flexion, the waist knee weakness, numbness, chills of thermophilic, joint tenderness, tinnitus, dizziness, fatigue, god joint pain, waist knee weakness, joint tenderness and so on. 6 ?The results of safety evaluation: The major adverse reaction of patients were gastrointestinal and liver damage, there were significant difference about adverse reactions compared(P < 0.05). There is no any adverse effect in patients in the treatment group.Conclusion: Firstly, clinical symptoms and clinical index of patients with osteoarthritis were improved significantly by Gubi decoction. Gubi decoction is an effctive and safe method to treat osteoarthritis and can be widely applied to clinic. Secondly, the content of MMP-3 and TIMP-1 and IL-1??TNF-? of patients with osteoarthritis were reduced. Regulating matrix metalloproteinase and inhibiting inflammatory factors may be the mechanism of Gubi decoction treating osteoarthritis.
