| Objective:To investigate the clinical efficacy of Dan Hong injection in the treatment of angina pectoris of coronary heart disease by performing a meta-analysis covering randomized con-trolled trials(RCT).Methods:We systematically searched Cochrane Library, EMbase, Pubmed, CBM,CNKI, VIP, WanFang Date, to search a RCTs of Danhong injection and Salvia miltiorrhiza injection in the treatment of angina pectoris of coronary heart disease. Retrieval time range from the inception of the database establishment to December 2014. According to the established inclusion and exclusion criteria, three researchers assessed the quality of the included studies by the Cochrane risk of bias,and extracted the information. Meta-analysis was conducted with the software Review Manager 5. 2. Three researchers independently completed the literature selection.Results:(1)Thirty-eight RCTs, including 3928 patients(without limit of race, age, or sex)with a course of treatment of 14—30 days. All the methods of clinical research were not rigorous enough, and the quality of literature was low.(2)Pooled analysis suggested that Danhong injection and Salvia miltiorrhiza injection showed significant difference in improving the total effective rate of clinical efficacy[RR= 1.30,95%CI(1.24,1.36),P<0.00001] and the rate of ECG recovery [RR=1.32,95%CI(1.25,1.39),P<0.00001]. Significant differences were observed in the improvement of the curative effect of angina pectoris[RR=1.27,95%CI(1.21,1.33),P<0.00001].Danhong injection and Salvia miltiorrhiza injection showed no significant differences in improving the Whole blood viscosity low cut value[SMD=-0.38,95%CI(-1.21, 0.46), P=0.38] or the whole blood viscosity at high shear rate values[SMD=-1.29, 95%CI(-2.56,-0.02), P=0.05]. No significant differences were observed in the blood lipids(TG) [RR=-0.21,95%CI(-1.48,1.05),P=0.74]or the blood lipids(TC)[RR=-0.61,95%CI(-1.32,0.10),P=0.09]. The effective of Danhong injection and Salvia miltiorrhiza injection in controlling anginal attacks frequency[SMD=-4.84,95%CI(-10.97,1.29),P= 0.12] as a secondary indicator were also similar.(3)The incidence of adverse reaction of Dan Hong injection and Salvia miltiorrhiza injection was low, and there was no significant difference between the two groups.(4)Subgroup analysis: In the improvement of clinical total curative effect, all kinds of dosage Dan red injection effect is better than Danshen injection, but the 40 ml dose effect is the worst. For ECG efficacy improvement, 20 ml and 30 ml Dan Hong injection were due to the effect of Danshen injection, but the 40 ml effect is not significant;The curative effect of angina pectoris was improved, and the effect of different dosage of Dan Hong injection was better than that of Salvia miltiorrhiza injection, but the effect of 20 ml dose was the best;The efficacy of Dan Hong injection in different course of treatment is better than Danshen injection, but 14~15 days of treatment efficacy is the best;20ml and 15 days can be prescribed for the clinical use of Dan Hong injection dose and duration of treatment.(5)Meta- analysis of adverse reactions:The difference between Dan Hong injection and Salvia miltiorrhiza injection group was not significant [RR=0.62, 95%CI(0.36,1.07)],But the trend is conducive to the treatment group, the treatment group than the adverse reaction was lower than the control group.Conclusion: Based on a meta-analysis of 38 randomized controlled trials including3928 patients, we found that Danhong injection can improve the total effective rate of clinical efficacy, the rate of ECG recovery, the symptom of angina and the blood plasma viscosity. Comparing with the Salvia miltiorrhiza injection used for treating angina pectoris,Danhong injection was relatively effective and safety,which was based on the systematic review.Limited by the quality and quantity of the clinical researches, the results of this study have lower intensitys. |
