The changes of Microcirculation between preoperative and postoperative of early stage necrosis of femoral head with core decompression combined with negative pressure were tested by DCE-MRI scanning quantitative analysis to evaluate its curative effect and analysis the mechanism of treatment. The study will provide a more effective method for the clinical treatment of early avascular necrosis.Patients with early stage necrosis of femoral head were brought into the study and with the operation of core decompression combined with negative pressure. The sample of people who've been followed in 1?3?6?12 months after operation with the hip X-ray examination and recording their scales of hip Harris and VAS to determine the function of hip postoperative,and in 1?3?6 months after operation with the hip DCE-MRI scan and detecting the relevant parameters quantitative of vascular function in femoral head to quantitatively analyze the changes of microcirculation in different regions of the femoral head in the preoperative and postoperative.At the time of the postoperative follow-up by X-ray and DCE-MRI scan showed not collapse or significant growth in necrosis region of necrosis of femoral head. The differences(P>0. 05) were not statistical significance that only exist in VAS score between the preoperative and postoperative 1 month, The differences(P<0.05) of the other compares in Harris and VAS score between the preoperative and postoperative different time were statistical significance. The differences(P<0. 05) in the ARCO stage ? group in necrosis region of comparing preoperative with postoperative 3 ? 6 months of Kep and comparing preoperative with postoperative 3 months of iAUC were statistical significance. The differences(P<0. 05) in the ARCO stage?group in necrosis region of comparing preoperative with postoperative 1 month of Ve were statistical significance. The differences(P>0. 05) in the ARCO stage?and ?group of the other compares of the four parameters of each district before and after operation different time were not statistical significance.This study preliminarily shows that core decompression combined with negative pressure is a kind of new therapy to treat the early stage necrosis of femoral head, and shows that the therapy to treat the disease can effectively improve the function of hip joint,reduce pain symptoms, and delay the collapse of the disease in the short term. This study preliminarily shows that the new surgical method to treat the early stage necrosis of femoral head is not only improving the microcirculation stasis of the ARCO stage?necrosis of femoral head,but for the ARCO stage II is not obvious, but also promoting capillary proliferation in the necrotic area and junctional zone. Because of the study was restricted by the sample size,the early stage necrosis of femoral head is treated with the new surgical method which lead to the situation of the clinical effect and the improvement of microcirculation stasis needing further large sample,long-term follow-up and pathological experiments before it can come to the conclusion that more accurate.
