| Background:Professor Li Yi Yi, is one of the fifth group of professors who are aim to let the traditional Chinese medicine academic experience be inherited. All of those professors are national traditional Chinese medicine experts. He engages in clinical medicine more than 50 years of dedicated research on TCM phlegm syndrome with rich clinical experience,He is especially be expert in treating coronary heart disease, cardiac arrhythmia,hypertension,stroke and other cardiovascular diseases from the view of phlegm and stasis,also have unique perspectives and higher attainments on treating diabetes gynecological diseases, skin diseases, diagnosis and other difficult diseases. During my three years of study with Professor Li, I had knowledge of his academic system of treating heart disease. In order to study and inherit the academic thoughts and clinical experience of Professor Li, I conducted the study.Objective:To study and arrange the stasis and phlegm academic thoughts and clinical experience of Professor Li on treating heart disease.Methods:Working along with Professor Li on paternity, Arranging summary of typical cases, writing notes about tips from Professor Li, Researching on Publications of professor Li, Retrieving relevant papers and books, Studding Chinese classics in depth, Asking Professor Li questions continually, etc. And observing the clinical efficacy of Fumai,the experienced prescription of Professor Li,on treating sinus bradycardia.Results:This paper can be divided into two parts, the first part is on induction, summary, introduction of the academic thought and clinical experience of Professor Li, so that to further explore the academic origins; the second part is on observing clinical efficacy of Fumai prescription on treating sinus bradycardia according with the norms of modern clinical research methods.The first part introduces the biography and clinical practice experience of Professor Li. Then discus the theory of Chinese medicine stasis and phlegm in terms of origin, various theories, clinical trials and other academic research respectively.Then focus on clinical experience of Li treating heart disease, and cite and analyze typical medical cases,Through to Professor li, sick sinus syndrome, coronary heart disease diagnosis and treatment of primary hypertension three common cause of heart disease and pathogenesis, syndrome differentiation characteristics were analyzed and a typical medical record, summarizes Mr Li's experience in the differentiation and treatment of heart diseases and laws, further to explore his academic thought, summarize the clinical experience.The academic thoughts of Profess Li include mainly: 1. Generation of Etiology and pathogenesis of phlegm and stasis syndrome. Professor Li treats qi-blood disharmony and fluid transport disorders as the basic pathogenesis of stasis and Phlegm.Physiologically,the body fluid and blood are being derived from the same source; Pathological, “Phlegm and stasis are homologous". Because of "the phlegm homologous blood", the formation of the occurrence of blood stasis and phlegm turbidity, positive phlegm material can achieve the purpose of for activating blood circulation.2. The treatment focuses on recuperating Qi and blood, especially on Qi function. Li believes Stasis is nothing but hurting their Qi and blood, regardless of causes, Qi and blood disorders is fundamental, those who are hurt their Qi and blood, blood stasis can occur. Therefore, the treatment should focus on adjusting Qi.3. Treating disease from the root, so focusing on spleen and kidneys. Li believes the Phlegm and Stasis is formed mainly with fluid metabolism that is directly related to the spleen, lung, kidney dysfunction, so pay attention to strengthen spleen and nourish kidney.4. Many Kinds of diseases are born in Phlegm, strange diseases should be treated as Phlegm. According to Li, "chronic illness will be stasis" and proposed that the Phlegm and Stasis is the common reason of many chronic diseases. Dissipating phlegm and removing blood stasis can play an important role in all those kinds of disease. Phlegm and dispelling stasis method through Professor Li always for the treatment of various chronic diseases as well as difficulty miscellaneous disease.Li pointed out: Visible blood stasis syndrome can be easily noticed, but for chronic diseases cause by invisible phlegm, those are hard to be treated, so it is a good way to treat them as phlegm and stasis. 5. The combination of Chinese and western, pay attention to scientific research. Professor li is in the modern Chinese medicine in early for reflection of the essence of phlegm turbidity, and combined with his own clinical experience and test observation puts forward some hypotheses about the nature of the phlegm and blood stasis. Professor Li think: cell function change may be the root cause of the phlegm turbidity, any abnormalities in the body's immune response activity product is likely to be associated with phlegm turbidity and blood stasis. Any abnormal biochemical reactions in the life activities of metabolites, synthetic material is likely to be associated with TCM phlegm turbidity. Professor Li think phlegm turbidity is the nature of the human body endocrine disorders caused by pathological metabolites.Li has rich clinical experience on treating heart disease with great effect.1. In term of treatment of coronary heart disease, Li believes that the coronary heart disease is deficiency in origin and enrichment in symptom based, the deficiency is in the heart-yuang, and the enrichment in symptom is stasis and phlegm. On the stage of attack, the main aspect is the enrichment in symptom, the treatment is focus on activating blood to remove stasis, clearing phlegm and stasis; on the stage of katabatic, and then the treatment is focus on invigorating Qi and warming Yang,nourishing kidney-yin. According to the current situation of modern physicians mostly lay particular stress on promoting blood circulation to remove blood stasis, li old think more should pay attention to qi and blood stagnation, phlegm and blood stasis stagnation.2.Li believes the sinus syndrome pathogenesis is poor in power on phlegm and stasis, lacking of heart qi, mind dystrophy. The clinical treatment focus on invigorating Qi and warming Yang and activating blood to remove stasis. Because of the variety of pulse manifestation, including late, knot and others, can be checked, He emphasizes particularly the usage of pulse diagnosis. On many cases, the sick is with organic heart disease, so the treatment should pay attention to changes according to real situation.3 Hypertension locates in the liver, roots in kidney, and the key is in the spleen. It is rooted in liver, spleen and kidney dysfunction, phlegm and stasis became as syndrome, emotional changes are more as an incentive. The phlegm and stasis also can be the result of Hypertension, along with the whole duration of Hypertension. Professor li in clinic with phlegm and remove blood stasis, flat liver extinguish wind as the main treatment, advocated the phlegm and blood stasis stagnation in hypertension treatment.At the last, the first part of paper cites and analyzes clinical medicines commonly used in clinical experience. The second part of the paper focus on observing clinical effects and security of Fumai on treated sinus bradycardia, with a view to provide better theoretical and clinical evidence on treating cardiovascular disease.Methods: Choose randomly 60 cases of sinus bradycardia patients, they wererandomly divided into treatment group and control group, each has 30 cases. The control group received Xinbao pill three tid. Experimental group received Fumai drink: Fritillaria 10 g, Trichosanthes 30 g, Xiebai9 g, Astragalus 30 g, leech 9g, Chuanxiong 12 g, Salvia 30 g, Guizhi 9g, licorice 10 g. Choose of Beijing Kang Rentang Ingredients recipe pills, one bag at morning and one bag at evening, half an hour after a meal with 200 ml boiled water. After 4 weeks of treatment, compare the average heart rate, the slowest heart rate, changes of syndromes before and after the treatment.Results: The average heart rate, the slowest rate, was significantly improved after 24 hours treatment(P <0.01), After treatment, the total efficiency of the average heart rate(93.33%) is better than the control group(86.67%), but no statistically significant difference between groups(P>0.05). The treatment group syndromes overall response rate(93.33%) is better than the control group(86.67%), the difference was statistically significant(P<O.05). Symptoms after treatment with comparison between groups, chest pain(P<O.05), with statistical significance.Conclusion: Fumai drink can effectively improve the heart rate and improve the clinical symptoms of patients with sinus bradycardia, the heart treasure pill has a considerable effect. But in terms of improving syndromes is better than Xinbao pill. |
