Objective:(1)To understand the epidemiological characteristics of hemorrhagie fever with renal syndrome in Gao'an city from 2005 to 2013,and explore the variation before and after hemorrhagie fever with renal syndrome vaccine inoculation.(2)To understand the hemorrhagie fever with renal syndrome vaccination status and explore the main influence factors in “16~60 years age” group.Methods:(1)Hemorrhagie fever with renal syndrome cases,rodent surveillance and demographic data of Gao'an city from 2005 to 2013 were collected.Descriptive epidemiological analysis was conducted on the epidemiological characteristics of hemorrhagie fever with renal syndrome in human and rodents,by which the regional distribution,seasonal distribution and population distribution before(2005 ~2008)and after(2009~2013)the vaccination intervention were compared.(2)200 permanent residents aged from 16 to 60 years old in Gao'an city were selected by multi-stage cluster sampling method.Then questionnaire survey on status and influences of hemorrhagie fever with renal syndrome vaccine inoculation was taken to them face to face.The total and whole course inoculation rates were calculated.Univariate analysis and Ordinal regression analysis were conducted to filter out the main influence factors affected hemorrhagie fever with renal syndrome vaccine inoculation.Results:(1)From 2005 to 2013,a total of 664 hemorrhagie fever with renal syndrome human cases were reported with the incidence density of 8.77 cases /100,000 population and the annual incidence in range of 6.96 / 100,000 to 12.19 /100,000.The total 23 towns all have reported cases,in which the incidence density of Yangxu town was the highest of 22.11 / 100,000,while Xinjie town took the lowest of 1.16/100,000.The annual incidence had a low peak in April-June and a high peak in October-January.Each age group all had reported cases,77.71% of which were in “15-59 years age” group.Incidence ratio of male and female was 2.27:1 and 71.08% of them were farmers.(2)From 2005 to 2013,the average annual density of rodents and rat virus index were 2.52% and 0.0208 respectively.The average annual density of rodents indoor was higher than outdoor(c 2=36.488,P=0.000),which were 2.21% and 2.87%respectively.Rattus losea accounting for 74.15% in field mouses,while Rattus norvegicus accounting for 88.38% in indoor mouses.(3)Compared to the period before hemorrhagie fever with renal syndrome vaccine inoculation(2005-2008),the incidence density of 15 towns(65.22%)suffered a decrease in the inoculation period from 2009 to 2013.The proportion of “16-60 years age” subgroups decreased continuously,from 81.40% in 2009 to 63.89% in2013.The annual incidence rate of “16-60 years age” subgroups declined significantly,while its border regions including Xinjian,Zhangshu and Yushui showed an upward trend.(4)The total and whole course inoculation rates of Gao'an city were 72.50%(145/200)and 56.00%(112/200)respectively.(5)Ordinal regression results showed that the high inoculation frequency in un-employees was 0.360 times for peasants(OR=0.360,95%CI:0.144~0.898);those who were not aware of widespread vaccination program was 0.079 times than who were(OR= 0.079,95%CI:0.033-0.192)and the high inoculation frequency of persons who did not care their own health was 0.073 times than who cared a lot(OR=0.073,95%CI:0.013-0.398).Conclusion:(1)The hemorrhagie fever with renal syndrome incidence of Gao'an city from2005 to 2013 was mainly in autumn and winter.The age of patients was mainly in“15-59 years age” group.The average annual density of rodents indoor was higher than outdoor,and the dominant species were Rattus norvegicus and Rattus losea respectively.(2)Hemorrhagie fever with renal syndrome vaccine implement has had a notable effect in HFRS prevention and control of 16–60 years of age subgroups.The increase of rat virus index might be the main factors due to the HFRS increase trend from2009 to 2011 after vaccine inoculation.(3)The total course inoculation rate(72.50%)reached the national requirements vaccination rate(70.00%)level,but the whole course inoculation rate(56.00%)needs to be improved.(4)Occupation,awareness of widespread vaccination program and careness of health were the main influence factors affected hemorrhagie fever with renal syndrome vaccine inoculation of targeted age(16–60 years of age)subgroups in Gao'an city.
