Object: To compare the clinical effects and safety of Laparoscopic and open surgery in treatment of rectal cancer.Methods: We searched the randomized controlled trials(RCTs)comparing laparoscopic surgery with conventional open surgery in rectal cancer,which was published from 1996 until now.The studies were screened according to the inclusion and exc Iusion criteria.After extracting the relevant clinical data Meta-analysis was performed by using Review Manager 5.3 software.Results: eventually thirteen articles were included into this study.The baseline characteristics of the laparoscope group were similar to those of the open surgery group.In regard to the surgery data,Compared with open surgery group,Although the operation time in laparoscopic surgery was prolonged by 27.07 min [95% CI,(16.33,37.82,p <0.00001),the amount of bleeding is reduced by 70.36 ml [95% CI(-97.91,-42.81),p <0.0001],in addition,the number of retrieved lymph nodes(p = 0.28),and circumferential resection margin positive rate(p = 0.99)was no difference in the two groups.with regard to the postoperative complications,There were no statistical differences in the intestinal obstruction,ureteral injury,anastomotic fistula,wound infection between the two groups.in the hospital stay time,laparoscope group were shorter than the open surgery group with 3.85 days.The 3~5 years survival rate after operation is same between the two groups.Conclusion: Laparoscopic rectal cancer surgery has the similar efficacy in The 3~5 years survival rate as the conventional open surgery,and the trauma is lesser,postoperative recovery is better.
